Pharmacy Support Specialist

The Pharmacy Support Specialist is an integral member of the interdisciplinary team providing telehealth services to Managed Long Term Support Services (MLTSS) patients in the CareBridge 24/7 Member Support Program. The Pharmacy Support Specialist provides support for patient interventions that optimize medication access, improve medication adherence, and promote improvement in health outcomes.

Responsibilities:

• Respond to referrals and tasks from team members promptly

• Complete accurate medication reconciliations

• Recognize and address medication adherence gaps

• Submit and complete prior authorizations for medications

• Assist patients with medication access and address cost issues

• Assist the pharmacist with daily operations as needed

• Participates in HEDIS and quality initiatives

• Perform other duties as assigned

• Outreach to patients to recognize and address pharmacy needs

Qualifications:

• 2+ years' experience in a healthcare environment

• Unrestricted license in state of practice

• Experience working with an interdisciplinary team

• Experience with electronic medical records

• Medicare and Medicaid experience preferred

• Works and communicates well in a team environment

• Excellent communication skills

• Highly organized and detail-oriented

• National pharmacy technician certification

• High School Diploma or GED

• Experience and knowledge of basic technology
