Pharmacy Support Specialist
The Pharmacy Support Specialist is an integral member of the interdisciplinary team providing telehealth services to Managed Long Term Support Services (MLTSS) patients in the CareBridge 24/7 Member Support Program. The Pharmacy Support Specialist provides support for patient interventions that optimize medication access, improve medication adherence, and promote improvement in health outcomes.
Responsibilities:
Respond to referrals and tasks from team members promptly
Complete accurate medication reconciliations
Recognize and address medication adherence gaps
Submit and complete prior authorizations for medications
Assist patients with medication access and address cost issues
Assist the pharmacist with daily operations as needed
Participates in HEDIS and quality initiatives
Perform other duties as assigned
Outreach to patients to recognize and address pharmacy needs
Qualifications:
2+ years' experience in a healthcare environment
Unrestricted license in state of practice
Experience working with an interdisciplinary team
Experience with electronic medical records
Medicare and Medicaid experience preferred
Works and communicates well in a team environment
Excellent communication skills
Highly organized and detail-oriented
National pharmacy technician certification
High School Diploma or GED
Experience and knowledge of basic technology
