Responsibilities

• Develop pharmacy clinical initiatives for various specialties including (but not limited to) Antimicrobial Stewardship and Transitions of Care

• Provide education to the healthcare team including pharmacists, nurses, and physicians through in-services, presentations, lectures, clinical pearl memos and competencies.

• Participate in daily multidisciplinary patient care rounds

• Participate in Performance Improvement, Drug Utilization Evaluation Programs, and various hospital committees

• Provide drug information to professional staff and patients

• Assist with pharmacy staffing as needed

• Responsible for compounding and dispensing medications and other pharmaceutical supplies, reviewing orders and prescriptions for appropriateness and patient safety.

• Work collaboratively with other members of the healthcare team to provide optimal patient care and patient service.

• Oversee drug usage and ensures drug therapy optimization.

• Educate healthcare professionals and patients with regard to medication use as well as serves as a preceptor to Pharmacy students.

• Report to the Director of Pharmacy.

Qualifications

Job Requirements

• Graduate of a college of pharmacy accredited by the American Association of Colleges of Pharmacy

• Five years of previous hospital experience

• Minimum 2 years of experience - ASP/ Regulatory affairs/ Clinical initiatives/Student preceptor

• Current Pharmacy License in good standing from New Jersey State Board of Pharmacy

Preferred Job Requirements

• Bachelor of Science in Pharmacy required and/or PharmD
• Antimicrobial Stewardship certification
