Summary:

Under minimal supervision of a Pharmacist, the Pharmacy Navigator/Liaison will work closely with physicians, medical staff, nursing, providing integrated support to patients. Works closely with patients to deliver a full continuum of medication adherence support.

Encourages patients of clinics to utilize Covenant HealthCare Pharmacy to benefit from our complete Health System integration. Given that the Pharmacy Navigator/Liaison may spend time in provider clinics to help implement centralized services, seamlessly integrating with the clinic team is critical. The Navigator/Liaison works with patients, families, providers and other staff to promote timely access to needed care, provide daily continuity of care coordination, and coach patients, families and care givers to understand the patient care plan and self-care management responsibilities. This person demonstrates excellent customer service performance in that his/her attitude and action are at all time consistent with the standards contained in the Vision, Mission and Values of Covenant and the commitment to Extraordinary Care for Every Generation.

Responsibilities:

As a member of the clinic team, work seamlessly alongside hospital clinicians to serve patients both face-to-face and over the phone Educate prospective pharmacy patients on how Covenant HealthCare serves patients and improves their overall care. Support outpatient retail and specialty prescription medication needs.

Help obtain Prior Authorizations for patient medications and services by collecting and submitting all necessary information Follows prior authorization procedures to ensure standardization of approval for all sites Maintains continued authorizations ensuring continued patient access Notifies patients of their financial responsibility and discuss payment arrangements Coordinates and communicates information to providers/clinical staff, patient in a timely manner Notifies patient of the prior authorization status and informs them of their financial responsibility Ensure that patients remain on track with medication regimens- outbound therapy/medication adherence check-ups; secures refill prescriptions. Communicate effectively with the centralized Pharmacy services team Help develop new processes to ensure smooth operations and patient care Identify medications requiring special storage and/or handling such as hazardous medications and/or refrigerated medication. Identify supplies necessary for select medications such sharps containers, needles/syringes, alcohol swabs, Band-Aids.

Answer, resolve and triage inbound calls and questions Make refill reminder calls under the direction of a pharmacist. Resolve insurance related issues, including prior authorizations Assist patients with access to medications through various forms of financial assistance. Ensure appropriate documentation in all required systems Apply observations and recommendations to operational issues, increased productivity, quality, and customer-service standards.

Identify trends, resolves problems; recommend improvements; implement change. Actively participate in process improvement initiatives. Keep skills and knowledge up to date through educational opportunities, professional publications, and participating in professional organizations. Other duties as assigned.

Other information:

EDUCATION/EXPERIENCE REQUIREMENTS • High School Diploma or GED required • Pharmacy technician who is registered as a pharmacy technician in the state of Michigan • Nationally certified as a CPhT or equivalent is strongly preferred • Preferred 3-5 years' experience as a Pharmacy Technician • Additional future licensure may need to be obtained in a mutually agreed upon timeframe. • Preferred experience working with electronic medical records (EPIC) KNOWLEDGE/SKILLS/ABILITIES • Excellent organizational, teamwork, interpersonal, time management, consensus-building, and written and verbal communication skills • Excellent communication skills (fluency in writing, reading and speaking English) • Proficiency with MS Word, Excel, PowerPoint, and other software • Ability to work with Electronic Medical Records and Pharmacy Dispensing Software • Ability to perform a variety of manual dexterity movements (i.e., keyboard data entry) WORKING CONDITIONS/PHYSICAL DEMANDS Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards Frequent standing, walking, sitting,use of hands/fingers, talking and hearing Occasional lifting, carrying, pushing and pulling, stooping, kneeling, crawling, twisting or reaching Occasional lifting up to 25 pounds Work within various settings within the organization including but not limited to- Outpatient Pharmacy, Hospital Floors, Physician Offices, Call Center
