Summary:

Reports to Residency Program Director Pharmacy Services. Under general supervision of the Pharmacist-in-charge or a Pharmacy Supervisor is responsible for the interpretation evaluation preparation and dispensing of medications pursuant to a legitimate order or prescription in keeping with established policies procedures standards of practice and scope of pharmacy services.

Collaborates with multidisciplinary care providers to monitor assess and intervene as needed to ensure safe appropriate and cost-effective medication use. Oversees and closely reviews the work of pharmacy technicians and interns as applicable.

Responsibilities:

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Provides safe and effective patient care to a diverse range of patients including those with multiple co-morbidities high-risk medication and multiple medication regimens.

Receives patient medication orders or prescriptions as written or ordered by a physician or other provider. Reviews and interprets each order to determine completeness therapeutic appropriateness of the patient�s drug regimen as it relates to the patient�s clinical status concomitant drug therapy and diet and with consideration of the aging processes and human developmental stages (neonate child adolescent adult geriatric).

Contacts the prescriber or covering physician as necessary to initiate appropriate interventions to ensure therapeutic efficacy minimize potential risks of drug therapy and optimize cost effectiveness.

Applies pharmacotherapeutic and pharmacokinetic principles to ensure safe and effective use of medications for the patient populations served.

Provides drug information and consultation to prescribers nurses and other health care professionals regarding appropriate drug therapy including drug interactions potential side effects incompatibilities allergies medication order discrepancies drug stability problems and drug administration. Proactively reviews and approves medication orders in the electronic medical record (EMR) including drug dose dose form frequency and duration of therapy. Ensures relevant patient specific parameters are present in the EMR and considered (i.e. diagnosis known allergies weight age etc.).

Supervises work performed by pharmacy interns and technicians to ensure performance standards are maintained and work is in accordance with federal state and organizational rules and regulations.

Identifies documents resolves monitors and prevents drug therapy related problems including contraindications duplications of therapy and drug interactions.

Engages with other clinical pharmacists or clinical pharmacist specialists for complex clinical situations. Demonstrates personal interpersonal and teamwork skills during the provision of care.

Integrates patient-focused medication therapy management and pharmaceutical care in accordance with established clinical pathways or guidelines.

Participates in prospective and retrospective pharmacokinetic dosing and monitoring of selected medications and recommends dose adjustments and laboratory monitoring as required.

Manages formulary and medication-use processes effectively.

Ensures compliance with target drug programs formulary management initiatives medication management initiatives and adverse drug event monitoring programs.

Documents and reports all clinical interventions medication errors near misses and adverse drug events in accordance with established protocols.

Participates in emergency code blue response as applicable.

Attends and participates in daily medical multi-disciplinary and specialty rounds as time allows.

Reviews and approves formulations for sterile and non-sterile medications requiring compounding or manipulation prior to dispensing. In accordance with established procedures completes in-process and final product checks prior to dispensing medications prepared by pharmacy technicians.

Participates in continuous quality improvement activities and total quality management initiatives including but not limited to adverse drug reaction pharmacist clinical intervention and medication incident reporting programs.

Provides effective medication and practice related education to health care professionals students patients and/or caregivers. Demonstrates sensitivity to the perspective of the patient caregiver or healthcare colleague in all communications.

Participates in the training of healthcare professionals including student interns and technicians physicians practitioners nurses and other care professionals on matters relevant to medication use.

Participates in coordination of transitions of care as appropriate.Removes barriers to medication access.

Maintains professional competence through participation in Pharmacy staff development and skills enhancement programs including but not limited to ongoing independent studyand membership and participation in professional organizations.

Demonstrates effective personal interpersonal and teamwork skills.

Performs other duties as assigned.

Other information:

BASIC KNOWLEDGE:

Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of pharmacy required.

Successful completion of ASHP accredited PGY1 residency program.

Prior to start date possesses current unrestricted licensure as a licensed pharmacist or as a 90-day temporary pharmacy license in the State of Rhode Island.

Prior to October 1st(or November 1stif the resident is granted a 30-day extension by the Resident Advisory Committee) of the residency year obtains unrestricted licensure as a licensed pharmacist in the State of Rhode Island then possesses and maintains current unrestricted licensure as a licensed pharmacist in the State of Rhode Island.

By November 1stof the residency year must possess and maintain current American Heart Association (AHA) Basic Life Support (BLS) Certification. Additionally if applicable and assigned must possess and maintain current AHA Advanced Cardiac Life Support (ACLS) Certification by November 1stof the residency year.

EXPERIENCE:

Clinical experience knowledge and development commensurate with a Bachelor of Science in Pharmacy or Doctor of Pharmacy degree.

Knowledge of area/specialty specific technology including but not limited to pharmacy information systems automated dispensing technology software equipment and automation. Experience with MS Word Excel Outlook and web browsers.

Effective communication skills both written and verbal.

WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:

Extended periods of time spent standing and walking.

Requires the visual and manual dexterity to operate a computer.

Occasional travel between facilities or sites as required.

INDEPENDENT ACTION:

Performs independently within broad scope of department policies and practices.

Refers specific complex problems to manager where clarification of departmental policies and procedures may be required.

None.
