Your Job:
The primary purpose of the Clinical Pharmacist is to perform professional, clinical tasks that will ensure the highest quality of pharmaceutical care and results in an optimal patient care setting.

Your Job Requirements:
Graduate from an accredited school of Pharmacy.
Current license as a Registered Pharmacist by the state of Texas - REQUIRED
Your Job Responsibilities:
Can perform the duties of a staff pharmacist when necessary including order entry and medication order interpretation.
Conducts DUE's, ADR documentation, writing of newsletter articles, target drug programs, involvement in critical pathways, and involvement in investigational drug programs.
Continually develops a general knowledge base of drugs and drug therapy in order to perform more effectively as a patient care and drug information provider.
Actively participates in departmental education activities.
Actively participates in professional organizations.
Knows the procedures and functions of areas relating to patient care and is able to assist in any drug distribution area when needed, including the IV lab.
Participate actively on a hospital or departmental committee or team.
Performs and documents clinical activities such as interventions, pharmacokinetic monitoring, and antibiotic streamlining.
Provides drug therapy management and consultation to medical STAFFnd other professionals on the provision of drug therapies and drug protocols with regard to age and disease specific populations.
Retrieves, evaluates, and disseminates drug information to other health care professionals and patients.
Consults with physicians, nurses, and allied health personnel regarding drug related issues.
Reviews Medication Administration Record (MAR) against patient profile information for completeness, allergy conflicts, and appropriateness of dose for patient parameters, duplicate drug therapy, and all drug interactions.
Contacts and confers with the physician in a manner that results in an effective outcome when questions, irregularities, or problems are encountered.
Serves as a preceptor/mentor for students and/or residents on clinical rotations. Provides timely and meaningful feedback to students/residents. Actively participates in department and/or continuing education program to enhance preceptor/mentor skills.
Performs any special projects assigned by management team.
