This position involves all aspects of the medication use process, including procurement, prescribing, compounding, dispensing, storage of medications, administration and monitoring. Partners with physicians and other clinicians to provide treatments and advice to patients, records patient history relating to medication use and ensures the safe and secure storage of medications and related supplies. Advises physicians in determining the best possible treatments for specific diseases, stays current on pharmacy regulations, treatments and developments in research and design. Pharmacists are the experts on drug therapy and the primary health professionals who optimize use of medication for the benefit of the patients. Meets educational needs of patients, healthcare team, students and residents. Ensures adherence to regulatory and fiscal requirements.
RESPONSIBILITIES
Patient Care
Provide comprehensive clinical pharmacy services to a specialty patient population on a daily basis, including attendance at medical team rounds, selection and dosing of medication therapies, therapeutic drug monitoring and adjustment of drug therapies. Additionally complete population based drug use evaluations and performance improvement initiatives (order sets, Epic alerts, guidelines, protocols, etc.) to improve outcome and safety for the specialty patient population. Provide 24/7 on-call coverage for pharmacy clinical services to the assigned medical team.
Teaching and Education
Serve as a preceptor and resource to the Pharmacy Residents, Pharmacists (Pharmacist I, Pharmacist II and Pharmacist III), pharmacy interns and students. Present accredited continuing education programs to the Division of Pharmacy and assist in the development and implementation of clinical therapeutic educational opportunities of the division. Coordinate inservice education of medical and nursing staffs for assigned patient care area.
Performance Improvement
Participate in improving organizational performance and patient safety through identification and implementation of improvement initiatives as well as participation on committees and improvement teams for their specialty patient population and the division of pharmacy.
Research/Scholarly Activity
Actively participate in the development, implementation and ongoing coordination of research projects and/or share knowledge and information through posters, regional or national presentations, or publications
Expected Starting Salary Range: 57.80 - 75.36
EDUCATION/EXPERIENCE
Required:
PharmD Degree
Pharmacist License in the state of Ohio
PGY1 Residency
PGY2 Residency in pediatrics or relevant patient population specialty
