Responsibilities:

• Coordinate and oversee all clinical pharmacy services, including medication therapy management, drug information, and clinical consultations.
• Develop and implement clinical pharmacy programs to improve patient outcomes and safety.
• Collaborate with healthcare providers to develop evidence-based treatment guidelines and protocols.
• Provide clinical guidance and support to the pharmacy team.
• Ensure compliance with regulatory requirements and industry best practices.
• Educate and train pharmacy staff on clinical pharmacy topics.
• Participate in quality improvement initiatives and medication safety programs.
• Stay updated on new developments in clinical pharmacy practice and contribute to the implementation of innovative practices.
• Maintain accurate and complete patient medication records.

Requirements

• Doctor of Pharmacy (PharmD) degree from an accredited institution.

• Pharmacy licensure in the state of practice.
• Minimum of 3 years of experience in clinical pharmacy, preferably in a hospital or healthcare setting.
• Strong clinical skills and knowledge of evidence-based medicine.
• Excellent communication and interpersonal skills.
• Ability to work effectively as part of a multidisciplinary team.
• Experience in developing and implementing clinical pharmacy programs and protocols.
• Familiarity with relevant regulatory requirements and industry guidelines.
• Attention to detail and strong problem-solving abilities.
