Austin Health Center Pharmacy (HJAP) Expansion Program Support

Sponsoring Organization: Henry J. Austin Health Center (HJAHC)

Project Name: Support for HJAP Strategic Goals

Project Number:

Project Period: 2021-2022

Background:

HJAHC will continue integration of clinical pharmacy services into the primary health care delivery model through efforts to expand clinical pharmacy services to improve community health outcomes and address health disparities worsened by the current COVID-19 pandemic. Expansion of Pharmacy hours, staffing and delivery services will maximize patient benefits from the 340B Program that enables covered entities to stretch scarce federal resources as far as possible, reaching more eligible patients and providing more comprehensive services. This is especially important as it ensures patients living in poverty get access to medication that would otherwise be too expensive and out of their reach. In addition, the Pharmacy will continue to develop sustainable expansion of HJAHC’s Medicare Chronic Care Management program. The CCM program brings evidence-based interventions that have been demonstrated to improve medication adherence and healthcare outcomes to medically- and financially-vulnerable populations that have been profoundly impacted by the COVID-19 pandemic. Finally, Quality metrics such as Uniform Data System (UDS) goals for Health Center patients nation-wide tell us that pre-existing inequities in health outcomes for patients living in poverty have widened dramatically during the COVID-19 pandemic.
