Practice Transformation is a primary care focused initiative. Our goal is to provide comprehensive care to our patients with the primary care provider acting as the "quarterback" in order to improve the health of our patients and our community.

POSITION SUMMARY Designs and provides ambulatory comprehensive medication management services for a subset of high priority patients designated at the clinics. Responsible for reviewing patient's medical records, medication lists, and pharmacy fill reports.

Conducting a review with the patient to cover medication adherence, understanding of the medication and schedule, therapy goals, adverse effects, and creating an Action Plan along with the patient. Providing patient education and importance of medication regimen compliance and follow-up. Communication with the patient's PCP and documentation from all reviews and communications to the patient's chart.

Promotes high quality, evidence-based, cost-effective pharmaceutical care. Performs these duties in the outpatient clinical areas assigned.

LICENSES/CERTIFICATIONS REQUIRED Licensed as a pharmacist in the state of Arkansas
