Job Title: PharmacistLocation: PruittHealth Pharmacy Services -Valdosta, GAPOSITION SUMMARY:Provides pharmaceutical services to patients receiving care in post-acute, long term, or retail settings in a manner that delivers superior patient care and excellent customer service.
Monitors and manages the prescription process and daily activities of the closed door long term care pharmacy in accordance with company, state and federal policies, laws and regulations. PruittHealth offers a 4 day work week for this position and it is a full time role.WORKING RELATIONSHIPS:Directly Reports to:Assistant Pharmacy ManagerESSENTIAL JOB FUNCTIONS, DUTIES, AND RESPONSIBILITIES: Review prescriptions issued by prescribers to assure accuracy and determine formulas or ingredients needed.
Evaluate prescriber medication orders for appropriateness of drug, dosage, potential interactions, route of administration and stability. Make recommendations for medication therapy changes to prescribers as appropriate. Collaborate with the healthcare team to provide clinical and technical support as needed.
Document all communication with physicians, nurses, and other members of the healthcare team. Direct and supervise ancillary staff engaged in packaging and labeling prescription orders. Responsible for accurate intake and efficient fulfillment of prescription orders.
Obtain, as necessary, and maintain all required documentation in accordance with federal and state regulatory agencies and Pharmacy Services policy. Implement policies and procedures for the care procurement, storage, distribution and use of drugs and biological, as well as disposal of drugs. Assist healthcare personnel with procedures relating to safe and effective methods of procuring, maintaining, administering and disposing of drugs.
Is a clinical resource and provides one-on-one drug therapy consultation with healthcare providers as required. Provide and document interim Medication Regimen Reviews for all new admit or readmit patients. Implement and document all approved therapeutic substitution medication orders.
Provide appropriate clinical assessment prior to contacting healthcare center. Demonstrate effective and professional leadership skills. Participate in pharmacy Quality Assurance/Performance Improvement activities.
Document and investigate errors and variances. Take ownership and accountability for the successful completion of all pharmacy production duties and achievement of pharmacy location goals. Periodically assesses the Framework LTC Batch Status Monitor and other automated tools to determine workflow, dispensing and toting needs.
This includes notifying other departments, (Order Entry, Pharmacists, Triage) of workflow needs. Follow all company policies and procedures, state and federal laws and regulations and is charged with reporting violations to the appropriate supervisor.ESSENTIAL SKILLS AND COMPETENCY:1. Outstanding telephone skills and etiquette.ESSENTIAL ADMINISTRATIVE FUNCTIONS:1. Participate in center/agency surveys (Licensure/JCAHO) and any subsequently required reports.2.
Attend and participate in continuing educational programs to keep abreast of changes in your field as well as to maintain current license/certification as required.3.

Graduate of ACPE accredited School of Pharmacy.2. Current licensure in the state of practice.TRAINING, SKILLS, AND EXPERIENCE REQUIREMENTS:1.
