Position Summary
The pharmacist is a part of the healthcare delivery team and is responsible for the use of medications throughout the hospital.
Minimum Qualifications
Graduate of an accredited School of Pharmacy
Maintains current pharmacist license in Oregon
Possess computer skills for order entry, word processing, clinical interaction
Experience with drug use evaluations/monitoring, clinical interventions
Effective communication skills with allied health care practitioners
Experience in sterile products preparation, including TPN, Chemotherapy
BLS and ACLS within three months of hire and maintain as current
Essential Job Functions
Modifies medication dosages as determined by age and weight, renal function
Correctly interprets/documents orders and intervenes on problem medication orders/drug interactions, ADRs, DUEs, errors
Maintains accountability within controlled drug inventory
Performs aseptic preparation of sterile products
Participates in clinical pharmacy initiatives, patient teaching, code support, effective use of dosing protocols, tracking of clinical interventions
Knowledge, Skills, and Abilities
Demonstrate sound judgment, patience and maintain a professional demeanor at all times
Ability to work in a busy and stressful environment
Organizational skills and the ability to prioritize
Computer skills: Word, Excel, Outlook, Electronic medical records software
Strong interpersonal verbal and written communication skills
Creativity, problem analysis and decision making
Ability to work varied shifts
