Pharmacist

At Select Specialty/ Regency Hospital, a division of Select Medical, our specialty hospitals care for chronically and critically ill or post-ICU patients who require extended hospital care. We are currently seeking a Pharmacist to join our team. We offer an excellent network of career opportunities across the U.S.

Responsibilities

Under the general supervision of the Director, the Pharmacist will be responsible for having a working knowledge of all areas of pharmaceutical services provided by Select Specialty Hospital, including drug distribution systems, central operations, and IV admixture.

As the Pharmacist, you will:

  • Provide clinical support of hospital pharmacy function in a fast paced environment

  • Maintains adequate drug supply at pharmacy at all times.

  • Work as part of the interdisciplinary clinical team

  • Correctly inform and instruct patients on any drugs to be used after they are discharged

  • Develops and participates in new employee training and education programs

  • Participates in patient drug therapy monitoring

  • Promote an environment of continuous quality improvement

Qualifications

Other requirements of the Pharmacist role include:

  • Pharm D or BS in Pharmacy required. Prefer Board Certification and/or Residency.

  • Licensed pharmacist with 3 years acute care hospital experience within the last 5 years

  • Ability to provide clinical staffing for the functioning of the hospital pharmacy

The Pharmacy Resident can expect to perform the following: * Conducts direct patient care activities using a consistent approach that reflects the philosophy of pharmaceutical care and is performed with the efficiency and depth of experience characteristics of an experienced pharmacist * Dispenses Medications: compounds and dispenses medications based on organizational and regulatory guidelines; selects ingredients for medications and determines pharmaceutical, chemical, physical or physiological incompatibilities * Review and interprets physicians medication orders with respect to the patients' medical record to evaluate untreated indications; improper drug selection; sub therapeutic dosage; over dosage; potential adverse drug reactions or interactions, and medication use without indication. * Designs, recommends, monitors, and evaluates patient-specific pharmacotherapy; * Documents pharmaceutical care activities appropriately; Uses an organized system for staying current with the literature; Solves practice problems efficiently; Recognizes appropriate situations requiring input from the senior practitioner; Assumes responsibility for the healthcare system's ongoing adherence to its medication use policies. * Participates in departmental projects and serves as a resource for information requests; * Provides concise, applicable, and timely responses to requests for medication-use education and drug information from healthcare providers, nurses, caregivers and patients; * Participates in healthcare system's formulary and drug use evaluation process; Participates in departmental formulary and medication use evaluation process, including working with the MUE(DUE)/ Formulary Coordinator in developing criteria, collecting data, and analyzing results of an MUE study; Participates in the hospital's or pharmacy department's continuous quality improvement process; Participates in research including but not limited to conceiving, designing, and completing a major research project; Participates in the healthcare system's process for assessing, managing, and reporting medication errors and adverse drug events.
