Pharmacist

Reporting to the Director of Pharmacy, this position is responsible for providing and coordinating pharmaceutical care services and collaborative drug therapy management for patients. The Clinical Pharmacist will serve as a resource to the patients, physicians, pharmacists, nurses and other members of the health care team.

Required Qualifications:

• PharmD - From an Accredited College of Pharmacy.

• Bachelor's Degree

• Pharmacy-Acceptable for employees hired prior to the revision date of this Job Description (10/01/12).

• California Fire and Life Safety Card

• National Organization within 30 days of hire.

• California Pharmacist Registered upon hire.

Preferred Qualifications:

• 2 years

• Acute care experience or a Pharmacy Residency equivalent to 3 years of pharmacy practice experience.
