Inpatient Staff Pharmacists are responsible for verifying inpatient prescriber orders; dispensing the ordered medications, compounding IV and all other sterile products, the timely preparation and delivery of all patient orders: providing drug information to physicians, nurses, patients, and other hospital personnel: and supervising technicians and interns. All inpatient staff pharmacists must also provide support on patient codes and emergency room level I traumas.

Demonstrates competencies defined in Inpatient Staff Pharmacist responsibilities in providing age-appropriate care to patients from the age of birth through 21 years and select adult patients.

What Will You Be Doing?

  • Dispenses inpatient medications pursuant to prescriber orders, including determining the appropriateness of medication orders using information such as patient age, weight, disease state(s) and pertinent past medical history, reviews all possible adverse drug events, and considers patient allergies.

  • Determines appropriateness of medication orders and contacts prescriber if in doubt of appropriateness of order. Verifies medications orders to assure timely and accurate delivery in a form appropriate for the patient.

  • Identifies potential adverse drug reactions, drug-drug, drug-disease and drug-food interactions, and incompatibilities affecting patient outcomes.

  • Demonstrates core clinical competency required to verify and dispense medications safely and accurately for patients (e.g therapeutic drug monitoring).

  • Provides information to health care practitioners, caregivers, or patients on the appropriate administration of medications, medication dosing, and medication selection.

  • Collaborates with other members of the pharmacy team to assure continuity of care and consistency of practice, including providing all necessary handoffs.

  • Collaborates on supply and inventory issues to ensure all necessary medications and supplies are available to accurately and completely treat intended patients.

  • Dispenses inpatient medications pursuant to prescriber orders.

  • Competently provides all necessary pharmaceutical care in support of inpatient codes and Level I traumas.

  • Makes effective use of available software and information systems

  • Supervises pharmacy technicians, interns, externs, and students in the performance of dispensing and compounding. Assures orderly workflow and provides input in performance evaluations. Participates in the interview and training of new employees when applicable.

  • Maintains all medication records required and in accordance with Federal, State, and Local law and regulatory standards.

  • Performs miscellaneous related duties as designated.

Qualifications

What Are We Looking For?

To fulfill this role successfully, you must possess these minimum qualifications and experience:

  • Doctor of Pharmacy (preferred) or Bachelor's Degree from an accredited college of pharmacy.

  • Hospital experience or internship desirable

  • Licensure, or licensure-eligibility, to practice pharmacy in the state of Ohio

  • Basic skills in using and documenting in an electronic medical record.

  • Strong communication and interpersonal skills required in providing education to patients/families and patient care team members; as well as in interactions with students, the public, pharmaceutical representatives, and representatives from external institutions (i.e., colleges of pharmacy, health care institutions).

  • Aptitude for detail and medication safety skills.
