Lake Health is seeking a pharmacist to participate in our decentralized pharmacist program at West Medical Center Stepdown unit. The pharmacist will be expected to spend 2-3 days per week in a decentralized role in the Stepdown unit, and 2-3 days per week in a staffing role in the department.

Lake Health pharmacists are an important part of the patient care team. They perform both distributive functions and clinical functions throughout the day.

Pharmacists review medication orders, make clinical judgments regarding appropriateness of therapy, and communicate with other health care providers regarding patient's medication therapy. They prepare, verify and dispense medications. Pharmacists participate in multidisciplinary patient care rounds with other health care providers.

They answer drug information questions, and share their knowledge with other health care providers and patients when needed. They assure that Lake Health is compliant with all state and federal regulations, guidelines of applicable regulatory agencies, and Lake Health policies.

Candidates for this pharmacist position should be eager to engage in the above opportunities and willing to grow with the evolving profession of pharmacy. This is a full time position, and candidate must be available to work in the decentralized role at West Medical Center and also in a staffing role first and second shifts, weekdays and weekends, alternating holidays at both West Medical Center and TriPoint Medical Center.

Essential Candidate Attributes: Solid critical thinking and clinical judgment skills as demonstrated by work experience, current knowledge of pharmacological therapy, demonstrate strong interpersonal skills and positive relationships with patients and coworkers.

Preferred Candidate Attributes: organizational skills, ability to prioritize, attention to detail, proficient computer skills, excellent written and verbal communication skills, previous hospital pharmacy experience or residency.

Education Requirements:
graduate from an accredited College of Pharmacy, Bachelor of Science in Pharmacy, or Doctorate of Pharmacy.

License Requirements:
licensed and in good standing with the Ohio Board of Pharmacy.
