Job Duties

Do you enjoy working with the public? The Western Tidewater Health District is currently seeking a qualified Public Health Nurse (PHN) to support Home and Community-based services provided by the Health Department.

The Western Tidewater Health District services the City of Suffolk, Isle of Wight County, Southampton County, and The City of Franklin. We assist in carrying out the VDH mission by detecting the needs of our community and providing quality care. The PHN Independently provides professional nursing care to patients in clinics, homes and schools and other community or congregate care settings.

The successful candidate will work primarily in the City of Franklin, Southampton County, and Isle of Wight and conduct home screening visits in partnership with Franklin, Southampton, and Isle of Wight's Department of Social Services. This position promotes public health and the well-being of Franklin, Southampton, and Isle of Wight residents, with a focus on the older adult population.

Duties include:

Displaying sound independent nursing judgements

Performing nursing assessments and administering case management

Serving on a team involved with community engagement across the Western Tidewater Health District

Supporting collaboration with community-based health care providers and community members Provide support to public health emergency planning and response.

The PHN will support activities focused on assessing and monitoring the public's health such as community health needs assessment and community health improvement planning. Must be knowledgeable of social and economic conditions related to public health and public health nursing.

Actions performed at this level require licensure as a RN by state code. May supervise support staff.

Minimum Qualifications

Requires licensure as an RN in Virginia or compact state. If a new graduate, must obtain RN license within 90 days of the date of hire or subject to termination per the regulations governing the practice of nursing.

Requires BLS certification. Requires adherence to all safety policies including a review of immunizations, driver safety policies and all other state policie

Communicates verbally and in writing, to analyze problems, situations and suggest solutions, leadership, follow directions and establish priorities

Skills in physical examination, health assessment, and interviewing and counseling technique

Ability to function independently, prioritize and organize work, solve problems, adapt to change, function as a team member and relate to the public

Knowledge of public health concepts, principles of public health nursing, principles of epidemiology; immunizations; basic nursing science, medical terminology, case management

May travel and some weekend and/or evening hours may be required

Must be certified in Basic Life Support (BLS)/CPR

Additional Considerations

Experience in a public health setting or one related to disease prevention and health promotion

Experience working with children and adults in outpatient settings

Family planning, immunization and sexually transmitted infection clinical experience

Experience with TB management
