Organized training program to advance resident's knowledge, skills, and abilities to become a more competent practitioner. Upon completion of the program, one shall develop competencies in areas of acute care, ambulatory care, medication management, practice management, and practice related education/training. The resident is also responsible for the efficient and accurate provision of pharmacist care including drug distribution.

MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1.Successful completion of Pharm.D. Degree from an ACPE accredited school of pharmacy.

2.Must possess current license to practice pharmacy in West Virginia or be license eligible. Licensure must be completed within 90 days. If licensure is not obtained within 90 days of the resident's start date, a customized plan of action will need to be created for the resident, to ensure that the resident can still successfully complete all requirements in a reasonable amount of time necessary to receive a Certificate of Completion. However, if licensure is not able to be obtained within 120 days of hire date, the RPD will notify the Resident that they are not eligible to receive a Certificate of Completion and their Resident contract will be terminated.

3.Obtain certification in Basic Life Support within 30 days of hire date.

CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. They are not intended to be constructed as an all-inclusive list of all responsibilities and duties. Other duties may be assigned.

1.Responsible for maintaining the pharmacy, utilizing time appropriately, and filling medication orders.

  • Consistently demonstrates the ability to keep up with workload.

  • Completes work left from previous shift, communicates with next shift to ensure continuity of service.

  • Adheres to controlled substances handling policies and procedures, including distribution, password control, and record requirements.

  • Assures proper storage conditions and security of medications in the pharmacy and on nursing units during unit inspection.

  • Regularly surveys nursing unit/clinic drug storage areas to ensure compliance with drug storage policies.

  • Strives to make the best use of time; minimizes non-essential interruptions.

  • Demonstrates the ability to prioritize activities.

  • Consistently demonstrates attention to detail and accuracy which includes the performance of computer order entry and review with a high degree of accuracy.

  • Documents and follows up on incomplete or pending orders.

  • Accurately fills prescriptions in a timely manner while following necessary regulations.
    2.Participates in educational activities related to drug therapy to enhance personal knowledge as well as that of other health professionals at West Virginia University Hospitals

  • Presents one continuing education (CE) program to pharmacists and one CE program to pharmacy technicians.

  • Provides the medical and nursing staffs with information regarding appropriate cost effective drug therapy and drug administration.

  • Provides drug information and educational programs to the medical and nursing staffs, other health care practitioners, and pharmacy staff.

  • Maintains professional competence by attending conferences, CE programs, and reading current professional literature.

  • Participates in the orientation and training of pharmacists, residents, technicians, and supportive personnel.
    3.Assures safe and effective use of drugs through clinical involvement

  • Participates in multidisciplinary rounds during clinical rotations.

  • Develops, recommends, monitors, and evaluates patient-specific drug therapy.

  • Assists in discharge planning and patient education during clinical rotations.

  • Participates in the management of acute and chronic disease states, medical emergencies, and adverse drug reactions.

  • Communicates relevant findings and recommendations to other health care practitioners.

  • Makes appropriate suggestions regarding drug selections (IV to PO switches when appropriate, offer formulary alternatives).

  • Documents relevant findings and recommendations in the current computer system.

  • Detects, appropriately documents, and helps to minimize or manage adverse reactions and medication errors.

  • Monitors protocol adherence as it relates to studies, critical pathways, and policy adherence.

  • Successfully completes clinical competencies required of staff pharmacists.
    4.Participates in administrative and quality assurance activities of the department.

  • Participates in the administrator on call schedule.

  • Prepares formulary monographs and presents to the P&T committee.

  • Directs the work of technicians and supportive personnel.

  • Communicates openly with technicians and support personnel.

  • Consistently demonstrates the ability to assess a situation and choose the appropriate course of action.

  • Ensures that technical duties are performed accurately and efficiently with quality results.

  • Ensures that policies and procedures are followed.

  • Provides ongoing feedback to technicians regarding their performance.

  • Routinely completes and submits written assignments in a timely manner, continuing education documentation, administrative staff minutes, and others that are assigned.

  • Performs other related duties as assigned.
    5.Responsible for drug use control in assigned service areas.

  • Assumes responsibility for and makes decisions related to the provision of pharmacist care in assigned service area.

  • Dispenses medication, including parenteral solutions, from the physician's order.

  • Reviews each order to verify and confirm accuracy and appropriateness of drug, dosage, route, and to detect incompatibilities or drug interaction.

  • Identifies problems related to aspects of drug use control and offers constructive ideas from resolution.

  • Ensures that policies and procedures related to drug distribution are followed
    6.Responsible for the completion of all Residency Program/ASHP Requirements as follows:

  • Successful completion of 10 four week Learning Experiences as determined by the Residency Program Director:

  • Required Learning Experiences (Ambulatory Care, Critical Care, Internal Medicine, Oncology, Pediatrics, Practice Management)

  • Elective Learning Experiences

  • Successful completion of a Medication Usage Evaluation and the presentation of its results in poster format at the ASHP Annual Midyear Meeting

  • Successful completion of a Research Project and the presentation of its results in a PowerPoint presentation at the ASHP Eastern States Residency Conference. This research must also be prepared into a manuscript style document.

  • Successful completion of all teaching requirements including CE presentation, facilitation of assigned PCL labs, and assigned IPPE facilitation sessions.

  • Successful completion of the assigned Pharmacy Practice Component (Staffing)

  • Successful participation and attendance to Journal Club

  • Successful achievement of 70% of all ASHP Required Goals/Objectives as determined by the Residency Program Director
