As part of the Telecare Family, House of Transition is a 15-bed facility, where adults with serious mental illness, ages 18-59, can receive help in transitioning into the community with an 18-month maximum-stay.
Employment Status: On Call, Per Diem
Under direct supervision of the Team leader, serves as the primary support and case manager for individuals with mental illness by assisting them to live as independently as possible in their chosen community.
High School Graduation or G.E.D.
Demonstrated sensitivity to multi-cultural populations and issues is required
Must be CPR, ProAct or CPI and First Aid certified on date of employment or within 60 days of employment and maintain current certification throughout employment
All opportunities at Telecare are contingent upon successful completion and receipt of acceptable results of the applicable post-offer physical examination, 2-step PPD test for tuberculosis, acceptable criminal background clearances, excluded party sanctions, and degree or license verification. If the position requires driving, valid driver license, a motor vehicle clearance and proof of auto insurance is required at time of employment and must be maintained throughout employment. If employed at Inpatient program, must assist with restraint of clients in the event of assaultive behavior and pass assault crisis/crisis prevention training
If employed at Outpatient program, must evade clients in the event of assaultive behavior and pass assault crisis/crisis prevention training
Establishes and maintains a therapeutic relationship with persons served
Make appropriate recommendations for living environments, therapy, psychiatrist visits, case management and other services
Provides services to all persons served as indicated by Service Plan
Document and communicate persons served interactions
Maintain awareness of community resources and assists persons served in appropriate use of community resources
Assists persons served with all needs, including ADL's Activities of Daily Living
Ability to link persons served to needed appointments
Assists persons served in obtaining appropriate benefits
Ability to advocate and accompany for persons served when needed to long appointments e.g. SSI, DMV, GR (General Relief) etc.
Ability to explain persons served their legal status and rights
Foster independence with persons served
Safeguards persons served confidentiality
Provides transportation when needed and appropriate
Provides safe, effective and efficient implementation of direct care in accordance with established policies, procedures and standards of care
Abides by and maintains compliance with Policy & Procedures

The physical demands here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
The employee is occasionally required to stand, twist, squat, kneel, crawl, climb, push, pull and lift and carry items weighing 25 pounds or less as well as to frequently sit, walk, reach, bend and do simple and firm grasping of up to 50 pounds. The position requires to drive and frequent Hazardous Exposure (Blood Borne Pathogens, hospital waste, Chemicals & Infections).
