This position is under the guidance and direct supervision of the PACE Supervisor. The Personal Care Attendant I provides support to participants across all platforms of delivery of care. Ability to assist in the coordination, preparation of all food services, support and assist center activities, coordinate the delivery of quality activity program/activities for all participants. Ensure that the recreational, physical, intellectual, spiritual, and ensures social needs of each patient is met. Accompany/assist PACE participants to and from outside specialty appointments.


1.Assist in the coordination of all food services including breakfast and lunch preparations, home meals, center snacks, and special events.

2.Assist with the day floor set-up, participant feeding, preparation of meal plates and snacks, serving of meals and cleaning as needed.

3.Assist in taking breakfast and lunch temperatures when the food arrives, checks and records temperature of food before serving. Informs dietitian of any issues.

4.Review, prepare, and update special diet lists and present trays as instructed; will use discretion to protect participant privacy as much as possible.

5.During meal service, utilizes posted diet lists or diet log, to ensure that modified diets and adaptive feeding equipment is provided correctly.

6.Help maintain records of meals served, participant lists, nutrition records, and other required reporting. Assist with tallies of all meals served.

7.Ensure proper safety techniques in food handling and serving.

8.Monitor kitchen supplies (plates, cups, creamer, napkins, and silverware), ensure they are full; replenish/replace and inform their supervisor about order placement.

9.Maintain cleanliness and order in the kitchen and food supply areas. Ensures cleanliness of refrigerators and freezers. Completes weekly cleaning log.

  1. Collaborate and assist Activities Coordinators by leading, conducting or supporting participation in activities in person, video or telephonically. Assists with daily tracking of the participant's participation in center activities, events, and outings.

  2. Assist with the maintenance, inventory, and management of activity equipment and supplies.

  3. Provide timely feedback to direct Supervisor, PACE Care Center Supervisor or Center Manager regarding participant satisfaction, change in behavior and participation.

  4. Escort participants to and from activities at the center, outside specialty appointments if needed.

  5. Assist participants with transfers and personal care while at the outside appointment.

  6. Provide all documents (I.e. Communication form, prescriptions) and orders to appropriate personnel at the center.

  7. Provide assistance with other departments at the center as directed by direct Supervisor.

  8. Attend meeting and training routinely and as needed.

  9. Perform all other related duties as assigned.


1.High school diploma or equivalent, required.

2.California Food Handler Certificate of Achievement upon employment preferred, must attain within 30 days of employment.

3.Minimum of 1-year experience working with the frail elderly (Geriatric) and disabled population, preferred.

4.Current BLS certification, required.

5.Bilingual English/Spanish/Cantonese/Mandarin, preferred.

The PCA II will perform and assist with personal care ADLs/IADL's for participants in the PACE across all platforms of delivery of care like center and/or in the home or virtual. The PCA II must be able to perform the job duties of a PCA I if assigned. In addition, the PCA II supports the PCA III in the gathering and reporting information related to the care coordination, participant's ability to engage, maintain/ improve their ability, foster independence, maintain function or improve socialization, or satisfaction. The PCA II communicates and collaborates with IDT members or other PACE staff in the goals of the plan of care, focusing on needs (like participant supplies or tools), and health status, physical and mental well-being. Responsibilities 1. Assist with assignments received from the PACE Care Center Supervisor and other clinical staff to determine needs for participant care and assist participants perform activities of daily living (ADL), (IADLs) such as bathing, dressing, toileting, eating and ambulating in PACE center, community and participant home settings. 2. Assist or reinforce messaging in the provision of health education and counseling to participant when indicated by the plan of care. 3. Provide assistance to the participant in obtaining medical services from providers, such as incontinent supplies. Order and ensure delivery of incontinence supplies to participants. 4. Assist in providing information, observations or direct experience that reflects a participant progress or abilities for the development and implementation of the nursing component in the plan of care in conjunction with the interdisciplinary team (IDT). 5. Assist in the provision of supportive and restorative nursing interventions as indicated by the treatment plan by engaging participants/patient during exercise. 6. Notify clinical staff and PCA III or direct Supervisor of any reported or observed change in participants' health status to facilitate further assessment of their condition. 7. Assist and/or oversee the collection of diet cards for each participant, meal distribution and arts/crafts distribution. 8. Perform all required documentation using appropriate tools, for medical record keeping accurately and in a timely basis. For example, record arrival and departure times, vital signs, gathering of data/information related to health status or condition and report appropriately. 9. Assist with coordination of all PCA related services to ensure compliance with Provider's, RN's, therapist's and nutritionist's orders. 10. Mandatory attendance and active participation in all education and training activities to ensure safe delivery of personal care assignment. 11. Assist PCA III in gathering information for the presentation of the participant's progress in the goals of their care plan. 12. Perform other related duties as assigned. Qualifications 1. Certified Nursing Aide education and training. Nurse Assistant Certificate by the State of California, Department of Health and Human Services - Aid and Technician Certification. 2. Current BLS/First Aid Certificate required. 3. High school diploma required or some college education/credits, preferred. 4. California Food Handler Certificate of Achievement upon employment preferred, must attain w/in 30 days of employment. 5. A minimum of 2 years' experience working with physically or mentally impaired older persons strongly preferred. 6. Bilingual: English/Spanish/Mandarin/Cantonese depending on location, preferred. 7.
