Job Description: Job Summary: Functions as a member of the health care team to assist the professional nurse in meeting the goal of individualized patient care by performing designated patient care activities. Performs additional acts that have been taught in present enrolled nursing curriculum. These additional skills must have been evaluated as competent to perform and be done under the direct supervision of a Registered Nurse. Essential Duties and Responsibilities include the following. Other duties may be assigned. 1. Communication. 2. Provides patient care activities that are routinely a part of any nursing assistant's job description 3. Environment 4. Performs professional responsibilities 5. Attends all required safety training programs and can describe his/her responsibilities related to general safety, department/service safety, specific job related hazards, and national patient safety goals. 6. Ensures positive customer relations interactions * Manages difficult or emotional customer situations * Responds promptly to customer needs * Solicits customer feedback to improve service * Responds to requests for service and assistance * Meets commitments Key Accountabilities/competency Key Accountability Weight 1. Provides complete verbal report with appropriate information. Effectively communicates to other care providers/team member's necessary information. Responds appropriately to customer requests. 2. Provides patient care activities that are routinely a part of any nursing assistant's job description including: Bathes patient. Assists patient in range of motion exercises (passive or active) Performs morning, afternoon and evening care. Positions, turns and ambulates patient. Assists patient with mobility needs. Changes bed linens of occupied and unoccupied beds. 3. Keeps patient room and hallways free of clutter. Disposes of soiled linen in appropriate container. Keeps nursing station clean and neat 4. Performs professional responsibilities. Accountable for accuracy and completion of all documentation requirements. Documents all care given, including patient preparation and patient response to treatment. Documents physical/emotional care rendered with patient response and/or outcome. Demonstrates fiscal responsibility through appropriate use of hospital resources. 5. Communicates any pertinent data in regards to patient condition to appropriate RN. Functions as team member, assisting health care team as needed 6. Communicates with others in completing work assignments and accomplishing department/unit goals. Utilizes appropriate channels of communication 7. Performs catheter care. Performs perineal care. Uses lifts for patient transfers when indicated. Takes vital signs, including VAS#, and records in electronic medical record. Makes observations hourly. Frequency may increase dependent upon patient status and unit standards. Measures and records weights 8. Clears IV pump on noncritical drips. Empties drains, catheters, drainage tubes. Records intake and output. Collects, labels and routes specimens. Performs pulse oximetry as directed. Performs oropharyngeal suctioning as indicated. Assists patient with incentive spirometry. Reinforces patient to turn, cough, and deep breathe. Feeds patients as indicated. Provides nourishments as directed. Passes fresh water every shift and as needed 9. Assists patient with elimination needs. Prepares and administers enemas. Incontinence care. Performs care established colostomies and ileostomies. Removes Foley catheter. Application and monitoring of restrictive devices. Initiates and follows High Risk Fall Precautions as indicated. Follows Suicide Precautions when applicable. Performs Finger Stick Blood Sugars. Provides skin care. Prepares patients for physical exam and assists when directed 10. The nurse extern, under the direct observation of an RN, may: Perform skills that have been taught in their nursing education program provided they are educationally prepared and clinically competent to perform the acts (example: placement of Foley catheter, placement or D/C of NG tube), draw blood specimens via phlebotomy as long as solutions are not injected and be assigned various performance improvement projects. The nurse extern may place IVs under the direct observation of the supervising nurse. 11. The nurse extern may not: Administer any medication, receive orders from a physician, independently assume the role, function, or responsibility of licensed personnel or substitute for licensed nursing staff 12. Attends mandatory in-services and unit meetings 13. Adheres to Nursing Department dress code 14. Maintains up to date Annual Core and Nursing Department/Unit Specific Competencies 15. Maintains up to date BLS certification Minimum Education, Experience, Training, and Licensures Required: High school diploma or GED required. Completion of first semester (in a 2 year program) of an accredited school of nursing. Completion of first year (in a 4 year program) of an accredited school of nursing. BCLS is required.
