The Perinatal Care Coordinator functions as a resource and advocate for pregnant and post partum women. Conducts maternal mental health screenings and systematically reviews screening data, provides appropriate responses for identified needs around social determinants of health, collaborates with all involved teams and partners to ensure that patient and family needs are met. Provides care coordination support for families experiencing difficulties getting needs met; ensures optimal patient coordination, excellent collaboration between teams, with a focus on family experience and outcomes. Minimum Education
Associate's Degree Associate's degree in social work, nursing, education, health sciences or related field is required (Required)
Bachelor's Degree Bachelor's degree in Social Work, Nursing, Education, Health Sciences or a related field is preferred (Preferred)
Minimum Work Experience
2 years Experience in a behavioral health or other health care setting as a member of a multidisciplinary team providing care coordination services to patients. (Required)
1 year Experience working with pregnancy and/or post partum patients, preferably providing care coordination services. (Preferred)
Required Skills/Knowledge
Excellent communication skills required.
Ability to work with diverse patient populations
Ability to provide culturally appropriate trauma informed care and communication
Knowledge of resources available to meet the diverse needs of families in Washington DC

Identifies patients/families appropriate for Care Coordination services in a systematic manner.
Completes ongoing staff education regarding coordination of care, culturally sensitive care, compassion fatigue, and other relevant topics.
Participates as part of a multi disciplinary team providing maternal mental health screening and response.

Follows through on any service needs or requests until complete and satisfactory resolution; updates the team as needed.
Prioritizes patient needs in the areas
Supports linkages between patients, the health care system and social services resources.

Completes documentation of all relevant conversations, meetings, and referrals.
Notes and, if appropriate, escalates patent concerns or needs to the appropriate person or service.
New Knowledge, Innovations, and Improvements
Participates in the research related to identifying the best maternal mental health screening and response tools and protocols for optimal patient experience and outcomes.
Provides regular updates to the program team regarding family support and patient status.

Updates team members as frequently as needed to ensure "one voice" consistent communication to families.
Acts as a liaison among the DC Mother Baby Wellness Initiative team, medical providers and social service partners
