Carries out Performance Improvement activities. Ensures that Performance Improvement activities are implemented according to NORTHWELL HEALTH Home Care Network Performance Improvement plan and process and are reflective of the Network's ongoing efforts to provide high quality service. Demonstrates and promotes service excellence at all times. Confers and collaborates with staff of Home Care agencies.
Monitors and evaluates the quality and appropriateness of patient care, resolves identified problems and, in collaboration with the Home Care staff, develops recommendations to improve patient care.
Collaborates with nurse educators and members of the management team to improve the quality of patient care.
Demonstrates working knowledge of the philosophy and concepts related to improving overall organizational performance.
Collaborates with other agencies/departments to coordinate aspects of Performance Improvement activities of Home Care.
Ensures a systematic process is utilized in monitoring practices' for quality patient outcomes.
Evaluates and takes appropriate action and reports the outcomes to the Performance Improvement Coordinating Group and the Administrative Director of Home Care.
Develops an age specific plan of care for a designated group of patients utilizing the nursing process of assessment, diagnosis, outcome identification, planning, implementation and evaluation of patient care.
Adheres to Infection Control practices and procedures.
Acts as a PI resources for the department.
Performs related duties, as required.
Bachelor's of Science in Nursing from an accredited college or university.
Currently licensed and registered as a Professional Nurse in the State of New York.
Minimum of two (2) years experience as a Registered Nurse in a Home Care environment. Previous experience in Performance Improvement/Staff Development activities or programs, preferred.
