Job Description

RESPONSIBILITIES

    1. General core privileges:

Obtains, documents and updates medical history. Identifies health risks and needs.

Performs physical examinations and conducts preventive screening procedures based on age and history. Formulates a differential diagnosis and plan of care based on history, physical examination, and clinical findings.

Identifies needs of the individual, family, or community.

Orders and interprets diagnostic tests. Identifies, prescribes, and administers pharmacological therapies.

Determines effectiveness of the plan of care through documentation of patient care outcomes. Reassesses and modifies plan of care as necessary to achieve medical and health goals.

Participates in research process, administers investigational therapy, and applies evidence based practices.

Makes referrals to other health professionals and community agencies. Admits/discharges to appropriate service.

Discusses advanced directives/end of life issues. Signs death certificates, as necessary.

2.Operational responsibilities:

Participates in team meetings and conferences to enhance an interdisciplinary approach to primary health care delivery.

Serves on departmental and multidisciplinary committees, which influence or determine policies affecting nursing practice.

Participates in and develops projects and unit-based quality improvement programs, as well as standards of care and practice.

Participates in approved medical and nursing research studies.

Maintains current level of knowledge relative to professional practice.

Maintains all required professional documentation, such as license, certifications, infection control, ACLS/BLS, DEA as well as system requirements.

Acts as resource to staff and implements educational programs.

REQUIRED EXPERIENCE AND QUALIFICATIONS

Masters Degree in a Nurse Practitioner Program, required.

Current license to practice as a Registered Professional Nurse AND a Nurse Practitioner New York State.

National Board Certification in specialty area, required. Certification to be obtained within ONE (1) year of hire. Board Certification is to match area of practice.

Has a collaborative practice agreement with designated protocols filed with NYS Department of Education OR collaborative relationship attestation as per the Nurse Practitioner Modernization Act 2015.

Previous experience relevant to the area of clinical practice, preferred.

Healing made personal.</p><p>Title: Per Diem Nurse Practitioner- Outpatient Oncology - MSH</p><p>Provides acute/primary health care in accordance with written policies approved by nursing and medical administration. Responsible for patient care as outlined within collaborative practice agreement. Develops and implements a collaborative practice agreement in the specific clinical area and performs a periodic review of the collaborative agreement.</p><p>Roles &amp; Responsibilities:</p><ul><li><p>Assesses the health status of patients/families by means of comprehensive health history and physical examination.</p></li><li><p>Initiates selected therapeutic and diagnostic procedures, consultations and laboratory examination, prescribes and alters medication regimens, interprets findings and alters plan of care according to collaborative practice agreement and protocols in designated area.</p></li><li><p>Educates and counsels in the areas of health promotion, maintenance, disease prevention, and management of acute/chronic illnesses.</p></li><li><p>Documents findings and plan of care in the patient's medical record.</p></li><li><p>Consults with physician as established in the collaborative practice agreement.</p></li><li><p>Performs a patient record review with the collaborating physician in a timely fashion but not less than every three months</p></li><li><p>Advocates for patients/families and supports patients' rights.</p></li></ul><p>Requirements:</p><ul><li><p>Licensure: New York State License with current registration as a registered nurse.</p></li><li><p>Master's degree program designed to prepare a registered nurse to provide acute/primary health care services or the successful completion of an approved supplemental education program, i.e., post-Masters NP certificate.</p></li><li><p>Is a graduate of New York State registered Nurse Practitioner Program or equivalent OR is certified as a Nurse Practitioner by a national association via examination.</p></li><li><p>Has completed a pharmacy component of not less than the required semester hours or the equivalent AND has been instructed in New York State and federal Laws relating to prescriptions and record keeping (3-hour course).</p></li><li><p>Has a collaborative practice agreement with a physician and designated protocols, both filed with the New York State Department of Education.</p></li><li><p>At least two years of nursing experience in area of specialty practice applied for.
