ABOUT THE POSITION

The County of Riverside

  • Riverside University Health System is currently recruiting for Per Diem Licensed Vocational Nurse I/II - Discharge Coordinators in the Integrated Case Management division. The individual in this position works under the direction of the RN and/or Social Worker to complete referrals for post-acute services for patients. Prior acute hospital inpatient case management experience preferred, excellent organizational skills, excellent verbal and written communication skills, people-oriented and compassionate, demonstrate problem solving skills and computer literacy.

Case Management and Utilization Review experience highly preferred

Work Schedule: 8 hours shift/40 hours a week (required to work weekend upon request)

Evening shift: 3:00 pm

  • 11:30 pm

Day shift: 8:00 am

  • 4:30 pm

Per Diem personnel are distinguished from regular employees in that they receive additional compensation in lieu of County fringe benefits and work assignments are flexibly scheduled or on an as needed basis.

EXAMPLES OF ESSENTIAL DUTIES

Coordinates discharge plans as directed by the case manager, i.e. placements, SNF's, Sub Acute, Home Healthcare/Durable Medical Equipment Skilled Nursing Facilities, Acute Rehabilitation, Long Term Acute Hospital.

Coordinates the progress of the discharge services requested with case managers, patients, families, physicians, other care providers and vendors to facilitate discharge plan and find necessary resources.

Notifies receiving facility of insurance information as needed, and processes referrals post-discharge.

Effectively communicates on the progress of the discharge services requested. Appropriately schedules, reschedules, cancels or confirms patient appointments in a timely manner to maximize scheduling for patient volume and in accordance with established authorizations.

Performs basic reception functions to ensure smooth office operations, including answering incoming calls, maintaining an accurate filing system, copying forms and documents, etc. Effectively uses computer programs to obtain demographic information and register patients, accurately verifies insurance coverage, including Medical Group eligibility. Accurately collects and documents all necessary and required information and activities.

Effectively communicates with patients and physicians to assure continuity of care. Appropriately provides patients with written and verbal instructions of physician's orders and requirements for pre-operative diagnostic -testing and/or follow up appointments.

Appropriately determines patient eligibility in relation to established program, insurance or contract standards. Responds effectively to questions and concerns expressed by patients and other staff and resolves identified problems (registration, eligibility, billing or financial) in a timely manner.

MINIMUM QUALIFICATIONS

Per Diem LVN I ($24.62 per hour)

Under supervision and direction of a registered nurse, nurse practitioner or physician, performs nursing duties within the scope of the Nurse Practice Act for vocational nursing, in the care of patients in a hospital inpatient setting or out patient clinic of a hospital, Public Health or Mental Health clinic; and to do other work as required.

Incumbents of this class are assigned to work in clinic assignments. Per diem personnel are distinguished from regular employees in that they receive additional compensation in lieu of County fringe benefits and work assignments are flexibly scheduled or on an on-call basis.

The Licensed Vocational Nurse (LVN) is not an independent practitioner. Licensed vocational nurses at this class level have less than one year of experience working as an LVN.

The Licensed Vocational Nurse I

  • Per Diem is distinguished from the Licensed Vocational Nurse II

  • Per Diem in that the latter have one year or more of nursing experience. The Licensed Vocational Nurse I

  • Per Diem is distinguished from the class of Nursing Assistant

  • Per Diem in that a state license is required of incumbents of Licensed Vocational Nurse I

  • Per Diem positions and they perform a more highly skilled form of nursing care.

License: Possession of a valid California Vocational Nurse's License issued by the California Board of Vocational Nurse and Psychiatric Technician Examiners.

Certification: Possession of valid Basic Life Support (BLS) issued by the American Heart Association for professional healthcare providers.

Per Diem LVN II ($26.08 per hour)

Under supervision and direction of a registered nurse, nurse practitioner, or physician, perform nursing duties within the scope of the Nurse Practice Act for vocational nursing in the care of patients in a hospital inpatient setting or outpatient clinic of a hospital, Public Health or Mental Health clinic; and do other work as required.

The Licensed Vocational Nurse (LVN) is not an independent practitioner. Licensed vocational nurses in this class have one or more years of experience working as an LVN. Per diem personnel are distinguished from regular employees in that they receive additional compensation in lieu of County fringe benefits and work assignments are flexibly scheduled or on an on-call basis.

Positions in this class are distinguished from the Licensed Vocational Nurse I

  • Per Diem level in that the latter perform work in clinic assignments with the Department of Mental Health, Community Health Agency or the Riverside County Regional Medical Center (RCRMC). Licensed Vocational Nurse II

  • Per Diem incumbents are either permanently assigned to work in an acute care assignment or they have full-time responsibility for directing and supervising staff and operations of a clinic. The Licensed Vocational Nurse II

  • Per Diem is distinguished from the class of Nursing Assistant

  • Per Diem in that a state license is required of incumbents assigned to the former class and they perform a more highly skilled form of patient care.

License: Possession of a valid California Vocational Nurse's License issued by the California Board of Vocational Nurse and Psychiatric Technician Examiners.

Certification: Possession of valid Basic Life Support (BLS) issued by the American Heart Association for professional healthcare providers.
