The Nurse Practitioner or Physician Assistant serves as a comprehensive direct health care provider at St. Luke's Rural Health Clinic, again named, "Pennsylvania's Rural Health Program of the Year". Located in Tamaqua, Pennsylvania. The CRNP or PA will provide primary and preventative care to our primary care/ family medicine patients. Fully integrated mental health services are provided on site by a LCSW. Our flexible schedule offers a great work - life balance. A typical schedule is 7am-3:30pm, Monday through Friday.
JOB DUTIES AND RESPONSIBILITIES
Initial evaluation, history and physical examination of patients whether they are inpatient or ambulatory patients.
Make decisions regarding data gathering and appropriate management and treatment for the initial evaluation of a problem or the follow up of a previously diagnosed condition.
Document patient contacts appropriately.
Perform therapeutic procedures as directed by a supervising physician.
Order or administer appropriate immunizations.
Provide counseling and instruction regarding common patient problems, as well as anticipatory guidance.
Assist the supervising physician as needed.
The advanced practitioner may execute and relay those medical regimens as dictated by the supervising physician and/or described in this written agreement within the supervising physician's scope of practice.
Provides patient care management within advance practice parameters.
Provides diagnostic and therapeutic measure.
Orders and interprets diagnostic studies.
Sets treatment goals and plans.
Evaluates quality of care and responses to therapy.
Collaborates and consults with other professionals; refers when necessary.
Provides counseling and support for families through the continuum of care.
Provides health teaching activities and resources for patient caseload with emphasis on wellness, health teaching and overall health lifestyles and/or adaptation.
Participates in community and professional efforts concerned with improving quality of health care in specialty area.
PHYSICIAL AND SENSORY REQUIREMENTS
Requires sitting, standing and walking for extended periods of time (up to 6 hours at a time). Continually uses fingers for patient care, writing, and handling patient charts. Routinely use upper extremities; occasionally lift items up to 25 pounds. Occasionally push cart with supplies up to 50 pounds. Occasionally push wheelchair with a patient weighing up to 325 pounds. Stoop, been, and reach above shoulder level regularly. Hearing as it relates to normal conversation and taking blood pressure readings. General visions, near vision, peripheral are all required.
Certified Registered Nurse Practitioner or Medical Physician Assistant with current license to practice in the state of Pennsylvania or New Jersey, as determined by the job location. Physician Assistants must also have a current certification through the National Commission on Certification of Physician Assistants (NCCPA).
TRAINING AND EXPERIENCE
One year experience in a similar setting is preferred. Current Basic Life Support (BLS) certification is required.
Days, M-F 7am-3:30pm with a potential for an occasional weekend or evening appointment to meet the special request of a patient.
