LifeStream is a behavioral health and social services organization that provides high-quality treatment, education, care management, rehabilitation, child welfare, primary care and homeless services to children, adolescents, and adults. Located in Central Florida, LifeStream primarily serves the residents of Lake, Citrus and Sumter Counties, with additional programs located in Orange, Marion, Hernando counties.

PEER SPECIALIST - CCBHC Citrus 120 JOB SUMMARY: The Peer Specialist (non certified) is a member of the multidisciplinary team has responsibility to carry out recovery rehabilitation and support functions that assist in treatment; provides job readiness training, provides education, support and consultation to families; assist consumers with goal setting and achievement; assists in facilitation of consumer connection with community supports and agencies. This position also provides crisis intervention under the clinical supervision of staff with a professional degree.

ESSENTIAL FUNCTIONS and RESPONSIBILITIES: Be a member of the team for an assigned number of consumers. Consult with community agencies and families to maintain coordination in the treatment process.

Document client progress to maintain a permanent record of client activity according to establish methods and procedures. Participate in daily staff organizational meetings and treatment planning review meetings. Participate in providing treatment and substance use services.

Participate in comprehensive assessment of psychiatric history (e.g., onset, course and effect of illness, past treatment and responses and risk behaviors), mental status, and diagnosis; physical health and dental health; use of drugs and alcohol; education and employment; social development and functioning; activities of daily living (e.g., self-care, living situation, nutrition, money management); and family structure and relationships The ability to comply with the regulations requiring a workplace of dignity and respect and equal employment opportunity compliance. The ability to work collaboratively with others and to accept constructive criticism from supervisors and managers. Willingness to abide by the company's published rules and regulations EDUCATION & EXPERIENCE:

Education:

High school degree/ GED Experience: Experience with adults who have severe and persistent mental illness Driving Requirements: Must be Over the age of 21 required Minimum of 5-years driving experience required Possess a Valid FL Driver's license (within 30 days of hire if currently do not possess) Possess a continuously valid US Driver's license for the past three years, from date of issue required Safe Driving record required TRAINING & DEVELOPMENT:

Completes all requirements according to Individual Training Plan within the first six months of hire and annually. Other training and/or professional trainings/ certifications.
