Description

Summary:

Under the supervision of the Clinical Supervisor-Care Management Services, the Peer Community Health Worker (CHW) will provide support at outreach and participate in community events.

CHWs will also assist with data entry, outreach, education, and other administrative tasks associated with the project. At events, CHWs will provide education, assistance, and community resources for mental health. Other duties would occur to support the project, such as data entry of registration forms, quality assurance, and other administrative tasks as assigned.

  • Reports to the Clinical Supervisor-Care Management Services or designee.

  • Establishes and maintains good working relationships with staff from other agencies and staff from Ibero programs.

  • Facilitates various outreach events, workshops, and activities for clients and families.

  • Maintains accurate records and progress notes.

  • Effectively engages and communicates with other Program staff to integrate client services.

  • Coordinate's activities and maintains appropriate schedules.

  • Conducts community outreach.

  • Conduct door-to-door outreach.

  • Wear appropriate PPE and follow infection control guidelines at event sites.

  • Complete data entry outside (as assigned based on CHW preference).

  • Work and collaborate with community partners.

  • Maintain high-level agency policies and procedures framework to work a flexible schedule.

  • All other duties as assigned by the supervisor.

Requirements

Skills and Abilities:

  • Lived experience.

  • A resident of Rochester with good knowledge of the city's communities and resources.

  • Experience in public health, healthcare, customer service, or a related field preferred.

  • Reliable transportation to and from work and work events.

  • Bilingual in English/Spanish preferred.

  • A combination of training and experience other than the specified, if judged to be adequate for the job, may be accepted by the President and CEO.

Physical Demands:

The position does require occasional standing, squatting, lifting of up to approximately 40 lbs. And frequent sitting.
