What You Will Do

As an Associate with Ascension Seton, you will have the opportunity to investigate, assess, and plan interventions to help patients cope with social, emotional, economic and environmental problems.

Responsibilities:

  • Interviews patients, assesses priorities and documents case activity.

  • Consults with team managers, statutory and voluntary agencies and patient's relatives to ensure compliance with patient's treatment plan.

  • Assists clients/families with obtaining community assistance by referral to proper resources.

  • Develops and maintains working relationships with community health, welfare and social agencies and seeks creative means to assist the client with his/her needs.

  • Supports management of concurrent review, discharge planning processes, social services, home care and other post discharge needs.

What You Will Need

Licenses/Certifications/Registration:

  • Required Credential(s):
  • Social Worker credentialed from the Texas State Board of Social Worker Examiners obtained prior to hire date or job transfer date.

Education:

  • Diploma

  • High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required

Work Experience:

  • 3 years of experience preferred.

In partnership with the multidisciplinary clinic team, the LCSW will provide direct therapeutic services to patients, including assessments and treatment plans. A typical day of the LCSW role entails spending approximately half a day conducting therapeutic services with patients and their families in a structured appointment format (30-45 minutes). The other half of the day will allow time for the LCSW to join primary care visits with complex situations as needed to support the provider. Furthermore, this time will also allow for follow-up on specific patient social needs, consults with other professionals or care team members.
