Primary Responsibilities:
As a part of Optum Care, the largest network of medical groups in the country, Reliant Medical Group is seeking a Part Time (0.8 FTE/32 Hrs) Advanced Practitioner to join our team in Shrewsbury, MA. Position Highlights
Will work with a team of 4 Physicians and 2 APC
Busy practice of about 6,215 members
The call will be via phone in the evening x1/month
Seeking part time between 32 - 36 hours per week
Performs age-appropriate physical assessments, evaluation, and treatment for primary care patient base
Covers primary care patients for same day sick visits
Order's lab studies when indicated. Identifies differential diagnosis. Assesses severity of patient's illness and establishes need/time for patient evaluation
Discriminates between normal and abnormal findings in the history and physical to establish a diagnosis. Performs tests and procedures as directed by diagnosis
Establishes age-appropriate plan of care related to patient's identified problem, including medication, diet, therapeutic procedures, etc.
Consults with other health team members regarding proposed treatment guidelines as necessary
Implements plan in safe and effective manner. Utilizes sound judgment in the establishment of priorities in patient's care and management. Records visit in concise, clear, logical manner
Evaluates need and/or time for follow-up and document
Makes and arranges appropriate referrals when necessary
Evaluates effectiveness of previously implemented medical treatment
Develops and provides educational materials and/or programs to patients, families, and healthcare professionals
Access to our pediatric extended care team on-site composed of a behavioral health provider, a medical social worker and a nurse care coordinator.

Required Qualifications:
Bachelor's degree
Graduate of an accredited nurse practitioner/physician assistant program
National Board Certification
Licensed by the Commonwealth of Massachusetts board of registration
Maintains BLS certification, professional certification as appropriate
2+ years of Pediatric Nurse Practitioner/Physician Assistant experience
