GENERAL SUMMARY/ OVERVIEW STATEMENT:

  • Care of the critically or intensively ill neonate in the neonatal ICU setting, including attendance at high risk deliveries with the NICU team, admission of the ill neonate to both the level III and level II setting, and ongoing care in the NICU/SCN setting.

  • The PNP/FNP shall work with a multidisciplinary team to review history and daily patient data, round with a multidisciplinary team to make management plans, perform appropriate procedures (including but not limited to: endotracheal intubation, needle thoracentesis, thoracostomy, paracentesis, umbilical line placement, PICC line placement, peripheral arterial access, lumbar puncture, bladder catheterization, RetCam exam eye screening).

  • The PNP/FNP shall maintain appropriate licensure and maintenance of certification.

  • Other duties as assigned.

GENERAL SUMMARY/ OVERVIEW STATEMENT:

The Pediatric/Family Nurse Practitioner (PNP/FNP) for the NICU is accountable for delivering comprehensive quality patient care, including diagnostic and therapeutic interventions according to standards of practice to premature, newborn, term or sick infants during the neonatal period. This person will function as a member of an inpatient physician team within an integrated academic model.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Care of the intensively ill neonate in the neonatal ICU/Special Care Nursery (SCN) setting, including, admission of the ill neonate to NICU/SCN, and ongoing care in the NICU/SCN setting.

  • The PNP's primary role and responsibilities will take place in the Growth and Development Unit (GDU), a subunit of our larger level III NICU where high-risk infants receive comprehensive, multidisciplinary family-centered developmental care. The GDU population consists primarily of (but not limited to) former very preterm infants born at or below 33 weeks' gestation, or other infants at high neurodevelopmental risk who have graduated the acute clinical stages, and are preparing for transition to home.

  • The PNP shall work with a multidisciplinary team to review history and daily patient data, round with a multidisciplinary team to make management plans, perform appropriate procedures (including but not limited to: intravenous line placement, peripheral arterial access, lumbar puncture, bladder catheterization, circumcisions, frenulotomy, gavage tube placement, RetCam screening eye exams). Additional evaluation and/or procedural skills for well newborn and convalescent infant populations at high developmental risk am appreciated.

  • Treat and evaluate patients with neonatal illnesses, prematurity, congenital anomalies, and other conditions that may place infants at high neurodevelopmental risk and promote the health of the child and family.

  • Function in the daily management of the NICU with on-site physician collaboration for patients requiring Level I-III care.

  • Respond to and appropriately manage life-threatening emergencies in the NICU/SCN/Nursery environments.

  • Analyze medical histories, diagnostic lab test and x-ray results and perform physical examinations.

  • Perform and maintain competence in required procedures, including neonatal resuscitation, lumbar puncture, circumcision, frenulotomy, and gavage tube placement, RetCam screening eye exams. Training can be provided to learn these procedures.

  • Consult with attending physicians for patient referrals and monitor outcomes.

  • Coordinate patient care with team members, other departments, subspecialty inpatient consultants at Boston Children's Hospital (BCH), outpatient care coordination with subspecialty colleagues at Massachusetts General Hospital or BCH or other, and community agencies and providers as necessary.

  • Function to identify, plan, implement, and evaluate the stabilization and emergency care of acutely ill neonates in collaboration with the attending physician, and arrange for management and/or transfer to higher level of care areas as appropriate.

  • Practice preventive health care through patient and family education and child health advocacy.

  • Participate in teaching and learning in an integrated academic model.

  • Precepting of APRN/PA Graduate students and mentoring of newly hired AP team members.

  • Demonstrates knowledge of the age-related differences and needs of patients in appropriate, specific gestation populations and applies them to practice. Demonstrates cultural sensitivity in all interactions with patients/families.

  • Demonstrates support for the mission, values and goals of the organization through behaviors that are consistent with the BWH Standards of Practice.

QUALIFICATIONS:

  • Masters' (or) DNP (or) PhD Degree in Nursing required. Satisfactory completion of an approved formal education program which has as its objective the preparation of professional nurses to practice as nurse practitioner. Completion of a Master's or post Masters' degree in Pediatric or Family NP program accredited by the Commission on Collegiate Nursing Education (CCNE) or the National League for Nursing Accrediting Commission (NLNAC).

  • Current Pediatric Nurse Practitioner or Family Nurse Practitioner board certification by an accredited APRN certifying body required.

  • Valid prescriptive authority from both MA DPH and Federal DEA required.

  • Current APRN License to practice in Massachusetts required.

  • Current Neonatal Resuscitation Program (NRP) certification required.

  • Minimum 2 year clinical nurse practitioner experience in a NICU setting or high risk infant setting required.
