The NP uses advanced skills in the evaluation, diagnosis and treatment of patients with diseases and adverse health conditions. The NP manages therapeutic regimens for acute and chronic problems associated with such diseases and conditions. The NP provides anticipatory guidance, patient and family education, and psychosocial and emotional support. The NP has prescriptive authority which includes Schedule II through V controlled substances in the Commonwealth of Massachusetts.The NP meets all requirements of the registered nurse's responsibilities which include assessment, planning, implementation and evaluation of nursing care for a complex patient population. Patients have a wide range of ages, acuity, and hematological and/or oncological diagnoses.The NP is a member of a multidisciplinary care team that requires effective communication and collaboration with co-workers and staff at the Institute and other collaborating institutions and agencies. The role requires critical thinking, an understanding of research principles and practices, and an expanded scope of clinical practice and leadership that improves the quality of care and advances the practice of nursing throughout the work setting and community. Responsibilities

Key Responsibilities The Pediatric Fertility Nurse Practitioner will be responsible for the following key aspects of the developing onco-fertility program for patients at the Dana-Farber/Boston Children's Cancer and Blood Disorders Center

Short-term responsibilities

  • Understand the patient's disease and risk of infertility to provide information relevant to their specific situation

  • Counsel Dana-Farber/Boston Children's Cancer and Blood Disorder patients and cancer survivors about fertility risks due to their cancer and/or cancer treatment

  • Provide information on fertility risks and feasibility of available and relevant treatment options to patients and families

  • Refer those interested in proceeding with fertility preservation/procedures to the appropriate reproductive specialists and other relevant providers at BWH and BCH

  • Be responsible for insurance authorization and assist with scheduling

  • Act as a liaison and resource for oncology/hematopoietic stem cell transplant providers and staff in BWH, BCH and Jimmy Fund Clinic

  • Develop and maintain an onco-fertility database to track patient outcomes and program quality metrics

  • Order and interpret diagnostic test results to obtain pertinent medical information, as indicated

  • Maintain clinical practice that is evidence-based and consistent with nursing policies, procedures and standards.

  • Adhere to the Code of Ethics for Nurses with Interpretive Statements (ANA, 2001) in all aspects of professional practice.

  • Foster an environment that is sensitive to the needs of diverse populations, including but not limited to culture, ethnicity, gender, and age.

Long-term responsibilities

  • Ensure comprehensive, seamless, and timely provision of fertility preservation services to patients and integration with their care

  • Update and maintain an onco-fertility resource website and educational materials (online and print)

  • Assist and develop onco-fertility evidence based practice, quality assurance/quality improvement and research initiatives

  • Act as a liaison and representative of the program with outside groups including other medical and nursing groups, donors, and the public

  • Participate in program evaluation and development of strategic goals.

  • If program expands, precept new team members

Qualifications

  • Licensed as a Registered Family Nurse Practitioner in the Commonwealth of Massachusetts

  • Masters degree required

  • 1 year Advanced Practice Nurse experience preferred

  • Certification in Oncology Nursing preferred - OCN, AOCN, CPON

Knowledge, Skills & Abilities

  • Expert knowledge of pediatric healthcare and preferable exposure to hematology/oncology/ hematopoietic stem cell transplant and/or fertility.

  • Must be professional, quality driven, and have the proven ability to work well within a cohesive group of peers. Exercises the highest level of professional judgment and will exercise independent originality in solving problems. Strong communication skills and sound judgment.

  • Possesses the maturity and confidence to work closely with clinicians, therapists, and team members across the organization. Ability to build working relationships amongst his/her peers and other team members working with pediatric and young adult oncology and hematopoietic stem cell patients.

  • Demonstrates a commitment to ongoing clinical education and training and exhibits expert knowledge of pediatric oncology/ hematopoietic stem cell transplant and fertility.

  • Able to accept instruction and guidance and assume graded responsibility in complex teamwork, while demonstrating an understanding of procedures, policies and documentation required to facilitate NP compliance with hospital standards of care.

  • Demonstrates ability to work collaboratively with all members of the care team and all DF/BCH staff.

VIEW JOBS 10/10/2018 12:00:00 AM 2019-01-08T00:00 GENERAL SUMMARY: To provide direct patient care and clinical assessment of adult spine patients under the direction of a specific physician who is an active member of the New England Baptist Hospital Medical Staff, Orthopedic Spine Section. These patients will be primarily patients with disorders of the spine. The Physician Assistant/NP will function within specified parameters as defined by the Supervising Physician, as well as the Chief of the Spine Section, Department of Orthopedics. PRINCIPAL FUNCTIONS: 1. Exemplifies the values of New England Baptist Hospital and the ROSE philosophy. 2. Complies with all New England Baptist Hospital policies and the Code of Excellence. 3. Maintains a safe and clean work environment. 4. Outpatient office responsibilities: * Will typically see patient in conjunction with sponsoring physician, but may be required to see patients independently. This may include initial patient evaluations, follow-up visits and postoperative checks. Will perform a history and physical examination, that is comprehensive or problem focused as appropriate. Will formulate a differential diagnosis and a working diagnosis; develop a treatment plan within the scope of practice and in conjunction with the surgeon. This would include implementation of therapeutic interventions for specific conditions when appropriate, and exercising judgment on conditions requiring consultation, referral, or evaluation by the supervising surgeon or other healthcare professionals. * Initiates appropriate laboratory tests, X-rays, and diagnostic studies. * Makes accurate identification of problems/diagnoses based on correct interpretation of findings. * Will work with patients to assist them reach informed decisions regarding their care, including providing appropriate education regarding their spine conditions and treatment options. * May be required to fit patients with spine braces in the office. * Will be required to use an Electronic Medical Record for documentation, when such a system is in use * Will write appropriate prescriptions for patients, * Will be responsible for completing paperwork generated in the course of patient care including, but not limited to disability and other insurance forms, and physical therapy requisitions * Will be responsible for returning patient calls regarding questions or problems, calling patients with radiological and other diagnostic results and other clinical information. Will also contact patients to assess clinical condition in the early postoperative period after discharge from the hospital, or when there are changes in their status. If matters cannot be disposed of promptly and satisfactorily, the supervising physician is to be involved promptly in determining disposition. 5. Assists Supervising Physician in surgery: * Responsible for knowledge of and adherence to all policies and procedures of the NEBH Operating Room. * Participates in those surgeries assigned by the Supervising Physician, acting as first or second assistant. * The supervising surgeon is required to be present at surgery, and personally perform all portions of surgery determined to be 'critical portions' by NEBH policy, including, but not limited to, obtaining localizing xrays, bony removal, decompression of nerves, placement of hardware, and fusion. * May be required, as skills permit, to close the deep and superficial aspects of surgical wounds 6. Will make rounds on in-patients of the Supervising Physician at the New England Baptist Hospital. * Will be primarily responsible for those patients on whom the Physician Assistant was First Assistant at surgery. This will include wound management, ordering medication, physical/occupational therapy, laboratory and radiographic tests, as well as coordinating with consulting practitioners * Will also round on other patients whose surgery the Physician Assistant did not participate in, to ensure that their care is in keeping with the Supervising Physicians general practice, and maintain contact with patients in anticipation of their post-discharge care. * Communicate plan of care to patient, patient's family, and nursing staff. * Promptly inform Supervising Physician about any significant developments in patient's status. * Communicate with primary physician the plan of care being initiated or discontinued. * Maintain current, accurate medical record documentation. 7. Miscellaneous: * Will occasionally be required to respond to calls for consultation directed to the Supervising Physician from all areas, including nursing units * May be required to dictate discharge summaries for patients of the Supervising Physician. 8. Education/Research: * Participate in the training and education of medical students and physician assistant students * Attend grand rounds in Orthopedics weekly when time allows. * Assist attending physicians with research and presentations as time allows. * Attend weekly morning lectures in spine as time allows. 9. Plans time away from work appropriately. * Arrives and leaves at designated times. * Will make an effort to plan, with as much advance notice as possible, any time away with the Supervising Physician to endure continuity of patient care. * Uses unplanned earned time appropriately (Based on full time hours. Standards are pro-rated for part-time employees based on agreed hours). 10. May be required to perform other clinical duties where the routines, scope or nature of the Supervising Physicians clinical practice change 11. Attends the annual safety review of the NEBH New England Baptist Hospital Boston MA
