Job Summary: The Pediatric Care Nurse Coordinator I organizes patient care activities and collaborates with the multidisciplinary care team to provide safer and more effective care.

This role serves as a resource and makes appropriate recommendations for patient care activities including monitoring, coordinating, and evaluating patient functional status; and providing support for the implementation of new policies, procedures, processes, and equipment. This role defines and resolves immediate clinical issues that promote the continuity of patient care ensuring high quality patient outcomes are accomplished that aligns with the goals and mission of the health system.

Education: Has completed the basic professional curricula of a school of nursing as approved and verified by a state board of nursing, and holds or is entitled to hold a diploma or degree therefrom.

Licensure, Registration and/or Certification: Valid multi-state or State of Oklahoma Registered Nurse License. Certification in area of specialty preferred.

Work Experience: Minimum 2 years of related experience.

Knowledge, Skills and Abilities: Working knowledge of general disease state management processes, bio- psychosocial model, and clinical pathways.

Demonstrates ability to be flexible and function effectively in stressful situations. Demonstrates ability to organize and prioritize work in an effective and efficient manner. Demonstrates ability to communicate with multiple disciplines to coordinate timely interventions.

Demonstrates ability to deliver high quality patient care to optimize patient safety and reduce the likelihood of medical/healthcare errors. Demonstrates ability to be a change agent with a high degree of commitment to create an environment where change can occur.

Essential Functions and Responsibilities: Provides clinical oversite of nursing care and finds resolution for issues; assists in providing care and interventions based upon the individualized plan of care by correctly assessing fluctuating patient and unit needs; continually assesses the effects of interventions, medications, and treatments.

Assists in guiding nursing staff in utilization of the nursing process and provides 'Just-in-Time' training to staff at the bedside. Works with healthcare team members, patients, and families to identify patients with risk conditions prompting the implementation of early interventions to prevent clinical deterioration; reports observations and findings regarding patient clinical status to the appropriate individuals and records these observations accurately and concisely in the patient medical record utilizing appropriate medical terminology and hospital-approved abbreviations. Facilitates communication between disciplines such as scheduling of urgent diagnostic tests and follows up on emergent diagnostic test results.

Acts as a positive leader, clinical care role model and innovator; collaborates with leadership and clinical teams to help develop individual care plans and interventions; monitors and evaluates current practice and standards of care and makes appropriate recommendations that fosters patient advocacy. Assures clinical care issues are communicated to the multidisciplinary care team, as needed; investigates patient care incidents and communicates findings appropriately and documents findings in variance reports. Completes health system, hospital, and unit specific competencies according to prescribed time frames; supports health system, hospital, and unit goals by attending staff meetings, in-services and educational offerings.

Decision Making: The carrying out of non-routine procedures under constantly changing conditions, in conformance with general instructions from supervisor.

Working Relationship: Coordinates activities of others (does not supervise). Leads others in same work performed (does not supervise). Has input on performance evaluations but does not prepare or give.

Works directly with patients and/or customers. Works with internal and/or external customers via telephone or face to face interaction. Works with other healthcare professionals and staff.
