About This Licensed Practical Nurse (LPN) Position for Hospice
HopeHealth is seeking an experienced LPN for its affiliate HopeHealth Hospice and Palliative Care to care for patients who reside in patient's homes. This position will entail seeing both pediatric and adult patients.
What will you do as a Licensed Practical Nurse (LPN)?
Provide direct patient care utilizing state of the art technology in collaboration with patient's RN Case Manager, physician and the interdisciplinary team
Connect with your patients and see the difference you are making in their lives.
Communicate regularly with the patient's interdisciplinary care team to ensure the most appropriate care is being provided.
Teach and educate your patients and their caregivers effective patient self-care techniques and approaches.
Follows physician orders and treatment plans for patients
Participates effectively as part of an interdisciplinary team to assure that patient services are delivered in a timely manner
Documents patient care and data accurately, thoroughly and by the end of the scheduled workday.
Secures positive clinical and quality outcomes according to state and national benchmarks.
Participate in weekend, and holiday rotations as needed.
Participate in case conferences.
What are the qualifications for this job as a Licensed Practical Nurse (LPN)?
Graduate of an accredited nursing school
Licensed as a Practical Nurse (LPN) in RI and MA.
Pediatric experience required
Minimum of 1 year clinical experience
Excellent clinical and communication skills
Dual Licensure in Rhode Island and Massachusetts required within 6 months of start date.
Experience in a Hospice or Home Care setting preferred
Experience with prioritization of care and care coordination
