This position is responsible for transporting patients to and from their bed and other areas such as therapy, surgery, or radiology. Other responsibilities include retrieving medical records, lab results, supplies and medications.

This position may be required to transport medications within their scope of practice and according to State Law.

This position is not responsible for providing care to patients.

Responsibilities

  • Transfers patients from one area to another by wheelchair, bed, or stretcher.

  • Assists in the transport of essential supplies during traumas and cardiac arrests.

  • Handles items such as IV bags, oxygen masks, oxygen tanks, urine drainage bags, blood bags, and medical monitoring devices that may be attached to the patient during transport.

  • Transports expired patients to the morgue by stretcher.

  • Initiates the call, upon patient discharge, alerting all departments involved in the bed turnover process.

  • Picks up and delivers routine and STAT laboratory specimens, blood and medical items.

  • Properly uses tele-tracking system.

  • Collaborates with other employees to ensure that the needs of the patient and department are met.

Qualifications Qualifications Required

  • High School diploma, or equivalency.

Licenses Required

  • Basic Life Support Health Care Provider card through American Heart Association or obtain within one month of hire.

EO/AA Patient Navigator/Social Worker for the University of Utah Transgender Health Program The University of Utah Transgender Health Program is a multi-disciplinary program that serves to provide and facilitate the heath care needs of gender nonconforming individuals. The Program includes specialists in family medicine, pediatrics, adolescent medicine, endocrinology, plastic surgery, urology, speech-language pathology, physical therapy, gynecology, fertility, and mental health. The Patient Navigator/Social Worker will have the primary responsibility of assisting patients in accessing care in the Transgender Health Program. They will be responsible for receiving and managing patient requests for information, and for coordinating care between departments and providers. They will perform patient intake interviews and work with staff in scheduling appointments, scheduling surgery, and coordinating pre and post op care. The will serve as a liaison for patients while hospitalized to ensure excellent patient experience. Additionally, the will assist the Program Coordinator with organizing meetings, outreach events, and education seminars. Typical hours will be 8am-5pm, although early, late, or weekend hours will be occasionally necessary. Transgender Health Program Responsibilities * Evaluates patients through interviews and psychosocial assessments in order to establish and develop treatment plans and treatment goals in conjunction with the licensed treatment team. * Administers approved counseling which involves social work assessments, counseling and/or treatment to patients, families or significant others. * Identifies and provides outreach in the form of information and referrals for community resources to patients and families such as shelter, transportation, future treatment plans and financial assistance. * Develops and provides stress management/wellness treatment plans for inpatient and outpatient populations. * May perform some Case Management duties. Knowledge / Skills / Abilities * Ability to perform the essential functions of the job outlined above. * Care is appropriate to the population served. * Demonstrated human relations and effective verbal and written communication skills. * Ability to work in a multidisciplinary team, problem solve, and resolve conflicts for individuals, couples and groups. * Ability to identify and arrange community resources that provide individualized assistance for patients, which may involve post-treatment financial assistance, shelter and transportation. * Ability to make recommendations based upon experience regarding policy and delivery of service. * Demonstrated knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. * Demonstrate the knowledge of the principles of life span growth and development and the ability to assess data regarding the patient's status and provide care as described in the department's policies and procedures manual. Qualifications Qualifications Licenses Required * One of the following * Current Licensed Clinical Social Worker (LCSW) certificate for clinical practice in the State of Utah. * Current license to practice as a Certified Social Worker (CSW) in the State of Utah. * Basic Life Support Health Care Provider card through American Heart Association or obtain within one month of hire. * Additional license requirements as determined by the hiring department. Required: * Must have valid driver's license with own transportation. Travel may be required. * Master's degree level mental health training * Experience in healthcare disciplines, ideally including care coordination, case management, community mental health, patient education and complex social issues, working with an adult and adolescent underserved population Qualifications (Preferred) Preferred: * LGBTQ community experience, including an understanding of biopsychosocial issues affecting transgender populations, such as healthcare needs, substance abuse, health risks, and legal issues. * Experience with electronic medical record (EPIC) Working Conditions and Physical Demands Employee must be able to meet the following requirements with or without an accommodation. * This position involves intermediate work that may exert up to 50 pounds and may consistently require lifting, carrying, pushing, pulling or otherwise moving objects while providing patient care to those requiring psychiatric care.
