Position Description

The Palliative Care PSR is responsible for scheduling all new patient evaluations, follow up appointments, answering phones and mapping routes for home visits.

Primary Responsibilities:

  • Schedules all new patient evaluations for Palliative Care.

  • Schedules all follow up appointments for Palliative Care Physicians, RN's and LVN's.

  • Responsible for mapping patient home visit driving routes for Palliative providers using a web based application (Google maps, Map Quest, etc.).

  • Completes intake for all new patient referrals.

  • Updates and maintains master list of Palliative/Bridges Patients for reports.

  • Updates and maintains SharePoint with all patient changes.

  • Answer phones, resolves call or routes to appropriate contact.

  • Manages medical records (maintains, files/scans, prepares for schedule).

  • Ensures all correspondence is scanned and/or filed in timely manner.

  • Processes requests for medical records release and maintains appropriate logs, etc.

  • Greets patients as they arrive and manages 20 minute wait time. (As applicable)

  • Assists patient with intake processes including copying required documents. (As applicable)

  • Collects co-payments, co-insurance and deductibles and issues receipts. (As applicable)

  • Manages cashier box and daily deposits according to company policies. (As applicable)

  • Processes walk-in patients and visitors. (As applicable)

  • Performs all other related duties as assigned

Required Qualifications:

  • High school graduate or GED required.

  • One to two years of related work experience including data entry.

  • Efficient with Microsoft Office applications to include Excel, Word and Outlook.

  • Experience with navigating Internet tools such as Google Map and Map Quest.

  • Working knowledge of medical office procedures and medical terminology.

  • Ability to accurately sort and file materials by alphabetic or numeric systems.

  • Effective communication and customer service skills required.

Preferred Qualifications:

  • ICD 9 and CPT coding experience preferred.

  • Bilingual (English/Spanish) language proficiency preferred

  • Working knowledge of EMR and TruCare.

  • Experience in a physician's office, clinical or hospital setting preferred.

(sm) The LVN / LPN Coordinator is responsible for successfully supporting patients with stable chronic health conditions to navigate the healthcare system. The LVN / LPN Coordinator assists in developing patient empowerment by acting as an educator, resource, and advocate for patients and their families to ensure a maximum quality of life. The LVN / LPN Coordinator interacts and collaborates with multidisciplinary care teams, to include physicians, nurses, pharmacists, laboratory technologists, social workers, and other educators. The LVN / LPN Coordinator works in a less structured, self-directed environment and performs all nursing duties within the scope of a LVN/LPN license of the applicable state board of nursing. Primary Responsibilities: * Works with the providers and clinic staff to identify patients with chronic disease diagnoses. * Supports longitudinal care of the patient with stable chronic care conditions by: * Performing focused assessment of chronic care conditions * Performing medication reconciliation * Conducting Motivational Interviewing and Self-Management Goal setting * Providing patient education using standardized, approved educational materials * Creating referrals to appropriate agencies and resources * Supports transition of the patient with stable chronic care conditions from inpatient to outpatient setting, by: * Performing focused assessment of transitional needs * Performing medication reconciliation * Reviewing contingency plan * Providing patient education using standardized, approved educational materials * Assisting with post discharge needs such as prescriptions, transportation, Durable Medical Equipment (DME), appointments * Notifies providers and/or supervising RN of changes to patient's health condition * Performs accurate and timely documentation in the electronic medical record * Participates in daily huddles and Patient Care Coordination (PCC) meetings * Prepares accurate and timely reports, as required, for weekly meetings * Maintains continued competence in nursing practice and knowledge of current evidence-based practices * May perform clinical tasks within their scope of practice * Performs all other related duties as assigned The pace is fast and you'll need to be comfortable managing multiple priorities.
