Responsibilities
The Home Care Scheduler's main function is to schedule and coordinate nursing and therapy visits for Stella Maris Skilled Home.
ESSENTIAL FUNCTIONS:
Coordinate the scheduling of new and existing patients in the software database and appropriately communicates scheduling additions and/or changes to patients and staff.
Responsible for promptly answering incoming calls and providing information and problem resolution in a friendly, professional and knowledgeable manner.
Assist in coordination of patient care with management and interdisciplinary team.
Monitors staff productivity and works with clinical managers as needed to assure efficient use of staff by utilizing full-time staff prior to scheduling part-time and per diem staff.
Ensures prompt scheduling of emergent visits and seeks the assistance of clinical mangers as needed.
Continually monitors phone, fax, e-mail and other electronic media for patient scheduling needs throughout the day.
Responsible for updating patient demographics.
Demonstrates commitment to excellent internal and external customer service.
Assures timely completion of individual staff schedules.
Performs general office duties including but not limited to word processing, photocopies, scanning, files, shreds, sort/distribute mail, provide reception and telephone services.
Requirements
High school diploma or equivalent
Prior experience in healthcare preferred
Demonstrated ability to work well with staff, residents and family members.
Excellent written and verbal skills.
Basic computer skills required
