This position reports to the Practice Manager of the MGH Chelsea Adolescent and Pediatric Clinic. The PSC III/Referral Coordinator is responsible for managing internal and external referrals. As part of that process, this person provides additional support for patients who require assistance navigating the healthcare system. The PSC-III/Rereferral Coordinator will also manage order-based scheduling for appointments within the practice. This position is part of a PSC team and will support other members of the PSC team and practice needs under the direction of the Practice Manager and PSC Team Leader, which may include cross coverage of the front desk, phones and other practice operations.
Duties:
Manages outgoing referrals, both within the MGH system and to outside organizations
Communicates with the offices/agencies to which patients are referred, establishing relationships with service providers
Tracks and appropriately documents information on referrals to ensure patient safety
Communicates with the care team regarding referrals - updates on appointments scheduled, unable to schedule, or no shows to appointments
Coordinates communication and information exchange with the Department of Children and Families with input from the care team when needed
Works with outside agencies for Pediatric & Adolescent Behavioral Health Rereferrals
Performs Early Intervention Referrals and tracking
Coordination of nebulizers
Respond to Patient Gateway Message Desk
Builds and maintains ongoing working relationships with staff, clinical and physician members of the practice to facilitate positive staff morale and support provider productivity
Checks in/Checks out patients and performs all task associated with check-in/check-out workflows
Schedules appointments, calls to confirm appointments as necessary, makes appointment changes as necessary
Manages office mail and faxes
Answers phones and clears voicemails on a timely basis
Performs all other duties and/or projects assigned by the Practice Manager and/or Unit Chief
Qualifications:
High school diploma or equivalent required.
Minimum of 3 years of administrative assistant experience, preferably in a health care environment.
Demonstrated knowledge of computer skills necessary to use appropriate modules/systems in IDX, LMR, US Bank, Micro soft Outlook, Nehenlite, electronic medical record modules, database programs and transition to future systems as required.
Must be fluent in Spanish and English.
Very good communication skills, both written and verbal.
Excellent customer service skills.
Ability to work well with Adolescents.
Ability to work with confidential and sensitive information.
Ability to deal effectively and courteously with a diverse group of internal and/or external customers; teamwork and flexibility
Good organizational skills and strong attention to detail. Ability to handle multiple tasks in busy environment
Ability to work independently within a team environment.
Able to problem solve and serve as a resource to other members of the team and take initiative to resolve complex issues on behalf of the providers and the patients.
Knowledge of HIPAA Confidentiality and Privacy Policies.
Schedule:
This is a 40 hour/week position, Monday-Friday 8:30am-5pm. This might include some evening and Saturday rotation as needed.
