Job Summary:

The Patient Safety Coordinator investigates significant safety and sentinel events and represents a key position in safety improvement and risk mitigation. Being a self-starter, having attention to detail, and having a passion for process improvement through causal analysis are key characteristics for this position. The Patient Safety Coordinator is responsible for supporting the hospital's Patient Safety program through focused reviews/investigations relative to patient/staff safety; facilitation of root cause analysis; presentation of reviews/investigations/root causes and associated data review and analysis.

The Patient Safety Coordinator:

Monitors enterprise-wide patient safety event reporting system (e-Safe)

Identifies trends within institution event reporting system.

Reports out trends within event reporting system to relevant stakeholders.

Facilitates Centers for Medicare and Medicaid Services (CMS)-related sentinel event investigations.

Facilitates apparent and root cause analyses of safety events, including interviews, HPI standard RCA meeting coordination, and document/record coordination.

Aids in initial and ongoing risk and patient harm reduction processes

Facilities education of staff on ACA and RCA processes and development of corrective action plans

Communicates outcomes from ACA/RCAs to organization, as appropriate.

Monitors action items from ACA/RCAs for completion

Validation of Serious Safety Event Rate (SSER) for Children's Hospital

Investigation of trends that may be contributing to SSER and escalation via appropriate chain of command.

Helps coordinate data submission to relevant Patient Safety Organizations (Press Ganey PSO, Child's Health PSO, Riskonnect)

Weekly report outs of case presentations to PSO (Patient Safety Organization) Safety Huddle for Children's Hospital at Erlanger

Accountable for Children's PSO hospital milestones and/or objectives for Children's Hospital

Collaboration with senior leaders and quality director for Senior Leader Rounds

Collaborates with Children's Executive Leadership for continuous maintenance and monitoring of Safety Huddles within Children's Hospital to meet SPS expectations and recommendations.

Assists in establishment and maintenance of Children's Hospital daily huddle/operational safety brief.

Assists in education and facilitation of Proactive Safety for Children's Hospital.

Monitors, maintains, updates, and facilitates Error Prevention program per SPS standards for Children's Hospital.

Facilitates collaboration with Children's Executive Leadership to assist in meeting organizational metrics established by the Child Health PSO (Patient Safety Organization).

Monitors, maintains, updates, and facilitates Safety Coach program per SPS standards for Children's Hospital

Collaborates with Children's nursing and medical leadership to facilitate engagement when patient safety risk trends identified.

Additional responsibilities may include projects to identify and manage improvement opportunities for key operational and clinical initiatives.

Education:

Required:

Associate Degree in Nursing or other health-related field.

Preferred:

Registered Nurse (RN)

Pediatric RN experience

Experience:

Required:

Three to five years of recent healthcare experience in an acute care or critical access facility.

Proficient in computer and internet skills, specifically MS Office Word, Excel, and Powerpoint.

Preferred:

Previous experience with MS Visio, pivot tables, other information and data analysis tools.

Experience with use of improvement science tools (e.g. PDCA/PDSA cycles, Key Driver Diagrams, Process Maps) desired, however department will train.

Experience with facilitation in process improvement (PI) projects.

Understanding of accreditation and regulatory requirements.

Familiarity with ISO 9001: 2015 criteria.

Experience with Electronic Medical Record chart review.

Works with clinicians, staff, and hospital leaders to develop processes and systems to promotes patient safety and embodies the hospital's core values in all interactions with patients, families, and colleagues.

Provides updates to leadership in review of adverse events, coordination of inquiries from external regulatory agencies and on-site investigations.

Identifies and accurately interprets significant trends and patterns in relation to patient safety and adverse events. Utilizes appropriate chain of command to escalate trends as warranted. Assists in providing appropriate situational awareness to appropriate key stakeholders.

Identifies system failures in relation to adverse events and works with clinical team(s) and hospital leaders to plan improvements to patient care and safety.

Leads clinical teams in developing and implementing practice and system changes designed to prevent errors, improve clinical care, and patient safety enhancement.

Assists in coordination and maintenance of the incident reporting system and programs including systematic review of all incident reports within provided scope filed for the purpose of identifying trends or patterns and develop recommendations for appropriate corrective action, prevention, training/education, and compliance monitoring.

Identify patient safety and facility risks, prepares institutional reports, coordinates all follow up and communication with external regulatory agencies.
