Under the direct supervision of the Manager of Eastern Iowa Region Crisis System and Care Coordination the Lead Care Coordinator will provide daily oversight of the program and staff to ensure quality benchmarks and contract obligations are met. Provide staff supervision daily to ensure that the care model and contract requirements are completed and that best care and patient outcomes for is provided each and every encounter. Provide support and education on crisis system services to all providers in the Eastern Iowa Region. Build relationships with law enforcement, first responders, and other key stakeholders.

Responsibilities
Program Level
Ensures that Eastern Iowa Region Crisis System employees meet contract requirements per month, quarterly and annually
Ensures that the program meets its quality benchmarks monthly, quarterly and annually
Monitors outcome measures weekly to ensure benchmarks are being met. Verifies metric accuracy.
Reports to Manager weekly, monthly and quarterly benchmarks.
Identifies process improvement initiatives weekly and bi-monthly to ensure programs are meeting financial and quality benchmarks.
Ensures that productivity levels are met monthly.
Monitors staff work daily to ensure assessment completion, quality and financial benchmarks are met.
Assesses and analysis patient care per staff to ensure benchmarks are being met: care model completion, quality benchmarks and productivity.
Identifies and recommends improvement opportunities.
Provides support to staff providing patient care to ensure the best outcome, every patient, every time.
Provides coaching, as needed, to staff.
Community
Builds relationships with hospitals, community mental health centers, substance use disorder providers and other community providers.
Build relationships with law enforcement, jail staff, Fire, EMS and Communications departments and provide support and education related the Eastern Iowa Region Crisis System services.
Provide community education on services available through the EIR Crisis System.

Knowledge/Skills/Abilities:
Must have interpersonal and communication skills necessary to deal with a wide variety of clients and professionals with the ability to negotiate and establish effective working relationships with a multidisciplinary healthcare and community service team.
Must demonstrate ability to identify and act upon psychosocial needs and work independently in an outcome orientated environment.
Strong leadership skills and ability to form professional working relationships with peers.
Ability to document in, have knowledge of and use of the computer.
