Social Worker, Care Coordinator, Co-Occuring

Full-Time + Benefits

M-F, 8-4:30

Under the direct supervision from the Director of Alcohol and Drug Abuse Services and with oversight from the Project Lead of the Robert Young Center Eastern Iowa MH/DS Region, the Co-Occurring Care Coordinator will participate in team delivered substance abuse care coordination services across multiple settings and all 5 Eastern Iowa Region Counties, i.e., hospitals, jails, treatment facilities, transitional living facilities and shelters, and family homes.

The Co-Occurring Care Coordinator will work in an integrated team including, CADCs, QMHPs, MHPs, nurses, physicians, County Services, Judicial and Corrections personnel. The Co-Occurring Care Coordinator may deliver services at the program site, in a community setting such as jail or county office, or in family homes. On-call crisis phone response required during working hours. The Co-Occurring Care Coordinator will have knowledge as it pertains to substance abuse services offered within the five county region and will complete assessments to determine level of care criteria as it pertains to substance abuse. The Co-Occurring Care Coordinator primarily works from 8:30 AM - 5:00 PM, Monday through Friday. The Co-Occurring Care Coordinator will be independently licensed, or, within 6 months from obtaining licensure.

Responsibilities

Key Accountabilities:

  • Provide direct mental health and substance abuse assessment, intervention and consultation to identified individuals, interact with patients, family, and support systems to reduce barriers and promote progression of care.

  • Be knowledgeable of all regional substance abuse facilities and services and assist in coordinating care for co-occurring individuals across the region.

  • Assist citizens with transition of care services and placements.

Qualifications

Education:

  • Bachelor's Degree in Counseling, Social Work, or Psychology, MHP.

  • Preferred Master's Degree in Counseling or Social Work with 2 years' experience and license eligible within 6 months. Substance abuse or co-occurring education. Domestic violence, sexual assault or human trafficking training. Adverse Childhood Experiences and trauma informed care training.

Experience:

  • Internship or prior work experience working with individuals experiencing substance abuse and mental health or behavioral health issues.

License(s)/Certification(s):

  • Valid driver's license.

  • Will be required to obtain Iowa CADC certification within 6 months

Knowledge/Skills/Abilities:

  • Understanding of substance abuse and mental health diagnosis and clinical treatment pathways. Knowledge of social-emotional development and social derminents of health. Basis understanding of common treatment medications.
