Patient Referral Navigator - Part-Time With Benefits

The Patient Referral Navigator accepts incoming patient referrals from within or outside Frederick Health. Reviews patients' needs and then connects them to appropriate community-based programs or services. Works closely within internal and/or external multidisciplinary team members to remove any barriers patients may have to accessing needed programs and/or services.

Required: Three to four years of prior experience in a healthcare office setting, clinical/vocational training and/or a Certified Medical Assistant is required with the ability to perform job duties with minimal difficulty, exercising independent skills and judgment making.

Additional education may be substituted for clinical experience. Must possess intermediate proficiency and accuracy in the use of software programs such as Microsoft Word, Microsoft Outlook, Excel, CRISP, and Meditech 6.1(EHR based system). Clinical knowledge must include medical terminology. Must understand and be familiar with State of Maryland medical insurance carriers, both commercial and state governed, and possess an understanding of medical insurance policies and procedural/clinical based guidelines.

Must be familiar with CPT and ICD-9 codes and understand participating versus non-participating agreements. Requires excellent communication skills (including verbal, written and human relation); professional telephone etiquette; the ability to clearly communicate information and ideas, and the ability to work professionally with a diverse population.
