Responsibilities

Utilizing a customer centered approach, meet with qualified applicants to complete a face- to face or over the phone assessment to determine diagnosis, appropriate facility and level of care. Manage the applicant, referent, family members or other stakeholder's questions or needs during the pre-admission process, including education on the treatment experience and patient expectations. Work collaboratively with other disciplines to ensure a smooth conversion from applicant to admitted patient.

  • Conduct assessments to determine program appropriateness and level of care recommendation.

  • Create a client/customer focused experience for all clients/referents/customers seeking services through the Hazelden Betty Ford Foundation.

  • Complete accurate and timely documentation in the EHR.

  • Communicate timely and clearly with clinical team and others.

  • Provide crisis management services in order to prevent reduce or eliminate emergencies.

Qualifications

Required Qualifications:

  • Bachelor's Degree

  • Minimum 2 yrs experience providing alcohol and drug counseling (supervised internship applies)

  • Minimum 1 yr experience assessments for all levels of care

  • Must be age 18 or older

  • Current State Licensure or Certification in Alcohol and Drug Counseling or equivalent in CA, FL and NY only.

This position is eligible for a $1,500 retention bonus ($500 paid after 6 months, the remainder after 12 months) LADC - Outpatient Chemical Health Counselor Position Summary: Full-time Outpatient Licensed Alcohol and Drug Counselor (LADC) position for a substance use disorder treatment program serving those with co-occurring disorders with Canvas Health, a non-profit community behavioral health center. This position provides a full spectrum of outpatient treatment services including assessment, group facilitation, individual and family sessions, and treatment coordination. It serves as an integral member of our chemical health treatment team in accordance with the State of Minnesota Department of Human Services Behavioral Health Division and 245G statutory guidelines. Primary services provided at the Chisago County Jail. Must be able to work flexible hours. We seek demonstrated ability to provide the highest quality of professional and customer service. Qualifications: Bachelor's degree in human services or related field preferred. Must hold a valid license for an Alcohol and Drug Counselor from the Minnesota Board of Behavioral Health and Therapy. Also three years' experience in the field of substance use disorder services with a minimum of two years direct experience that includes experience in delivery of cognitive behavioral approaches, motivational interviewing and working with co-occurring disorders preferred. Experience working with Mental Health Services, Court Services, Social Services and Managed Care networks required. Experience with Electronic Health Records preferred.
