Job Description

General Purpose of Job:

Assign admitted patients to the most appropriate level of care. Assist with special needs request for bed assignment.Triage One Calls and Emergency Department One Calls, obtain admitting and consulting physician(s) and assist with transportation as needed.

Essential Duties:

This job description is intended to cover the minimum essential duties assigned on a regular basis. Caregivers may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position.

  • Coordinates, prioritizes and assigns all patient admissions from Emergency Department, Post Anesthesia Care Unit, other internal departments, physician offices, outlying hospitals, etc.

  • Identifies attending physician for patient's entering from other facilities or offices. Set up conference call with appropriate physicians and caregivers.

    Communicate incoming transfers with all appropriate departments. Assist with special requests such as transportation, special equipment, additional testing, etc.

  • Screens for insurance requirements and provides notification of admission for required carriers.

  • Assists admitting source in ensuring that the patient receives the needed services in keeping with patient choice and payer coverage. If necessary, assists them in routing patient to another hospital within the patient's insurance network.

  • Enters required information into the Electronic Medical Record and/or One Call program.

  • Arranges for the internal transfer between patient beds and units. Manages special bed needs, such as critical care, telemetry, isolation, etc.

  • Attends and actively participates in the daily bed meetings to plan for potential and known admissions, discharges and transfers.

  • Prioritizes bed cleaning as needed for efficient patient flow.

  • Works collaboratively with Patient Registration, Emergency Department, Patient Support Services, Nursing, Environmental Services, Facilities, Transportation, Surgery, Cardiology, and other Sparrow Caregivers to ensure a smooth admission process for physicians and patients.

  • Participates in departmental Performance Improvement, Corporate Compliance, Patient Satisfaction, and other activities to ensure optimal operations of the department. Actively participates in the design and implementation of strategies to improve department operations and patient outcomes.

Job Requirements

General Requirements Registered Nurse with current Michigan license. Work Experience Minimum of five years recent experience in an acute care hospital in ED, critical care, med-surg, or nursing (hospital-wide) supervision or 3 years recent experience in an acute care hospital with recent charge experience on a regular basis, or two years previous experience in an Admission/Bed Placement Center at an acute care hospital of similar size and volume of admissions.

Experience as charge nurse, nursing supervisor or critical care nurse. Education Graduate of an accredited Nursing program Preferred-Bachelor's Degree in Nursing Specialized Knowledge and Skills Demonstrates excellent clinical knowledge, skills and judgment, with strong clinical nursing assessment skills.

Demonstrates excellent critical thinking skills.

Demonstrates ability to handle multiple tasks concurrently, think logically and sequentially, creatively problem solve. Demonstrates ability to quickly adjust to change. Demonstrates ability to work independently, organize and prioritize work to meet all deadlines.

Demonstrates excellent verbal and written communication skills, including relationship building, negotiation, persuasion, marketing and advocacy. Demonstrates ability to work in stressful situations, manage conflict and assume a leadership role. Proficient in use of computers.

This position requires computer testing. You must pass a typing test at 45 WPM or more and a cusomter service test at 90% or higher.

Specifically, this position will work closely with the Sparrow Medical Group (SMG) Senior Health Clinic (SHC) to assist patients and their caregivers in accessing the essential information and services needed to attain an optimal quality of life. The Resource Coordinator will be responsible for engaging the senior population and connecting Sparrow patients with community resources where needed. He/she will serve as the outreach coordinator, educator and conduit to internal/external services. Common Duties and Responsibilities: As a central component to the PHSO Navigation Hub, the Resource Coordinator will work closely with the Geriatric Patient Navigator to effectively manage Sparrow patients across the care continuum. The Coordinator will act as a support mechanism for the larger operations and appropriately work with/triage patients to the next needed level of care.
