I. Major Responsibilities:

1.Under the supervision of a Registered Nurse or Licensed Practical Nurse, and according to hospital and departmental policies and procedures, actively assists in the care and treatment of patients.

2.Attends to 1:1 or up to 4:1 observation that has been assigned by nurse. Conducts patient safety checks. Meets all observation requirements and accurately completes all documentation.

3.May assist patients with activities of daily living, feeding, personal hygiene, etc. escorts patients to bathroom as needed based on plan of care.

4.Collaborates with the patient's assigned nurse and performs environmental safety check of patient and patient's room and will remove all potentially harmful items at the beginning of each shift and documents completion.

5.Reports any potentially unsafe behaviors to the assigned nurse throughout the shift.

6.Follows the policy and procedure for Constant Observation and high-risk suicide. Accompanies the patient at all times during the shift unless otherwise ordered by the assigned nurse and or physician. May accompany the patient out of the room only after obtaining approval from the assigned nurse.

7.Ensures anything brought into room by dietary or visitors is allowed based on plan of care.

8.Maintains a safe environment for patients and their families, employees, and visitors by adhering to infection control requirements and safety regulations.

9.Documents patient care activities accurately and completely and according to hospital and departmental policies and procedures



  1. Assesses and responds to cultural differences of patients and their families and is sensitive to the values of those cultures.

  2. Exhibits a positive attitude and promotes a positive work environment.

  3. Assists in assigned department when observations are not required, including transport and stocking and other duties assigned by the resource nurse or Director.


Standard Staffing Level Responsibilities:

1.Complies with established departmental policies, procedures, and objectives.

2.Attends variety of meetings, conferences, seminars as required or directed.

3.Demonstrates use of Quality Improvement in daily operations.

4.Complies with all health and safety regulations and requirements.

5.Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients, and visitors.

6.Maintains, regular, reliable, and predictable attendance.

7.Performs other similar and related duties as required or directed.

All responsibilities are essential job functions.

II. Position Qualifications:

License/Certification/Education:

Required:

1.High School Diploma/GED

2.Successful completion of a Constant Observation Training Program and completes a written post-test and safety skills check.

3.CPI Certification required for all Patient Observers within 3 months of hire and annually thereafter.

Preferred:

1.Certified Nursing Assistant

Experience/Skills:

Required:

1.Must possess the following skills: reading and writing; integrity and flexibility; must be a reliable and competent person.

2.Ability to understand and follow through on instructions.

3.Ability to verbally communicate effectively.

4.Must possess strong observation and communication skills.

Preferred:

Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.

Department-specific competencies and their measurements will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents.

III. Physical Demands and Environmental Conditions:

Constantly: Working Indoors.

Frequently: Sitting, Standing, Walking, Bending, High Level of Stress, Precise Motor Skills required, Precise Hearing required, Precise Vision required; Lifting, Carrying, Pushing, and Pulling

  • Less than 10 pounds to 20 pounds.

Often: Pushing, Pulling, Twisting, Climbing, Balancing, Reaching, Grasping, Performing Repetitive Movements; Lifting, Pushing, and Pulling

  • 20 pounds to 100 pounds; Carrying

  • 20 pounds to 50 pounds.

Never: Kneeling, Crawling, Working Outdoors, Working in Temperature Extremes, Working at Heights; Lifting, Carrying, Pushing, and Pulling

  • Over 100 pounds.
