This position is responsible for providing access to care to patients diagnosed with HIV and/or, hepatitis C. The position bridges the gap between patient and provider, through regular communication with the health care team, serving as an advocate for the patient while linking them to their respective care needs. In order to be successful in the position, the ideal candidate will: * Serves as an internal linkage to care specialist and attempts to remove any barriers for the patient to access the full scope of services available for care related to HIV, hepatitis C, and other health conditions. * Schedule and relay information between patients and medical office staff. * Tracking patient appointments and attendance. * Assist in the compilation of screening-level data points in accordance with mandatory program reporting, including monthly, quarterly, and bi-annual reports. * Maintains sensitive, private health information in accordance to HIPAA standards and regulations. * Bachelor's degree in an allied health field, or a related discipline * Six (6) months of experience in a medical or clinical setting; * Or an equivalent combination of education and/or experience. * Ability to advise clients or community groups on health issues. * Ability to assess individual or community groups on health issues. * Ability to assess individual or community needs for educational or social services. * Knowledge of principles and processes for providing personal services. This includes client needs assessment, meeting quality standards for services and evaluation of client satisfaction. * Knowledge of the information and techniques needed to diagnose and treat human diseases. * Knowledge of administrative and clerical procedures and systems such as word processing, excel spreadsheets, managing files and records, designing forms, and other office procedures and terminology.
