General Summary:
The Patient Navigator is responsible for ensuring that Holston Medical Group patients who have accessed the Emergency Room in the past 24-28 hours have been contacted. They will assess the patient's needs for accessing the ER and document appropriately within the EHR. The Patient Navigator will outreach to all patients within 48 hours of hospital discharge, schedule follow up appointments with the PCP, give appropriate education on office hours as well as Urgent Care options, will offer same day appointments with the PCP and schedule appropriately.
Main Responsibilities:
Manage and maintain all inpatient discharges to collaborate care with PCP
Track and document all patient hospital discharges
Outreach patients by all exhausted demographics
Scheduling for all PCP's within HMG
Work in partnership with office staff for booking or overbooking opportunities
Manage, maintain, and collaborate for patient population for all HMG PCP locations
Track and document all patients who have accessed the Emergency Room
Track and document all patients who have a hospital discharge
Maintain Practice Management System appointments to ensure all outreached patients are given a timely appointment with their provider
Update patient demographics as needed
Manage and maintain accurate information in the patient's EHR record, ensuring integrity and quality of documentation
Tasking and appropriate documentation
Ensure appropriate records are available for appointment
Manage patient messages and transfer tasks appropriately
Set up files and logs for appropriate audit and tracking of all outreached patients
Review the attributed patient population as assigned, for any HEDIS gaps in care for the patient, such as, but not limited to diabetes and hypertension
Attest to measures which are documented within the patient's medical record, will outreach to the patient to discuss/explain the gap in care and schedule patient for appropriate follow up appointments to meet the requirements to close the gap measure and to meet the gap closure set forth by the insurance payer
They will greet and route in-person visitors for internal appointments at HMG locations
Education /Experience/Knowledge:
High School Education or equivalent required
Basic applications and networking skills
Skilled in communication outreach
Excellent organizational, communication, interpersonal, and time management skills with the ability to multi-task and prioritize own work
5+ years' experience in medical office operations
