State of Franklin Healthcare Associates is currently recruiting for a Part-Time or PRN Patient Navigator to work with our Facilities team.

The Patient Navigator will work under the supervision of the Facilities and Grounds Manager to welcome patients, direct and escort them as needed, and help to maintain clean common areas. Supports and adheres to the State of Franklin Healthcare Compliance Program, to include the Code of Ethics and Business Standards by following professional conduct, dress code policy, maintaining patient, provider and staff privacy and confidentiality. The candidate will have the ability to communicate effectively and courteously with administration, providers, patients and staff.
Essential Duties and Responsibilities
Greets patients and welcomes them to the clinic in a pleasant fashion with a warm smile.
Provides directions and escorts patients as needed. Takes great care in escorting patients.
Gets wheelchairs for the patient when needed. Always sets the brakes on the wheels chairs while loading, unloading and leaving a chair unattended. Can hold the wheelchair to assist the patient or family member. The Patient Navigator is not to lift patients into or out of wheelchairs or vehicles.
Escorts/transports patients within the building as needed.
Coordinates with van driver to arrange transport for patients to their vehicles.
Notifies Central Supply when leaving area unattended.
Drives employees, patients and visitors from and to their parking spots.
Greets and directs guests as needed.
Common Area Maintenance
Check the public restrooms in assigned area on an hourly basis. Ensure they are clean, stocked with supplies and free of water or other substances. Clean the restrooms as needed.
During poor weather conditions, report and/or clean floors (mop). Report the collection of snow, ice or water at any of the areas. Remove any debris or obstruction from entrances and sidewalks.
Ensures personal safety first. If a situation arises that appears threatening, calls for help and leaves the area. Does not go into a situation alone.
If approached by anyone requesting assistance in the parking areas, calls Central Supply for assistance. Takes down any information and calls it in.
Reports to Central Supply any activities that are unusual.
Locks and secures the exterior doors as required.
In Case of a Medical Emergency
Ensures the patient is safe.
Does not attempt to render medical care.
Calls for assistance (radio).
Goes to or calls the nearest clinic and asks for medical staff to come to the patient's aid.
Prior to leaving the situation, records the name of the medical personnel attending the patient, the patient's name, the date, time and description of incident. This information is kept on file in Central Supply.
Delivers and picks up mail throughout the building as required.
Maintains effective working relationships with patients, medical staff, office staff and the public.
Deals with stressful situations
Performs other clerical or maintenance functions as required by supervisor or physician.
Follows assigned duties outlined by specific practices. May be pulled to other areas of the building as needed.
Speaks clearly and concisely. Pleasant and professional demeanor and the desire to work with the public.
High school diploma or GED.

Position involves working both in and outdoors. Appropriate dress is required to handle the weather conditions. Patient Navigator is expected to stand for long periods, attend to patients as needed in the parking lot, assist patients in and out of vehicles and wheelchairs and walk distances within the building. Requires the ability to work under stressful or emergency situations.
