Overview

Position #: 8061 - 7:00am-7:30pm

  • Every other weekend
  • Provides general oversight of patient placement and flow throughout Capital Health (CH) through the utilization of the bed tracking system.

Responsibilities

  • Coordinates the functions of the Logistics Center and coordinates bed flow through the management of the electronic bed tracking board.

  • Provides daily operational oversight and decision making of patient flow activities throughout Capital Health.

  • Assesses patient status/acuity to facilitate the efficient movement of patients through the hospital system by prioritizing inpatient admissions, critical care admissions, and internal/external transfers.

  • Serves as an expert clinical resource for patient placement by ensuring smooth functioning of the Logistics Center process to reduce wait times and avoid unnecessary movements of patients. Serves as contact person for bed management as it relates to admissions.

  • Ensures throughput on units and compliance with transfer process through management of people and process.

  • Identifies and anticipates barriers to discharge/patient flow activities and works to ensure throughput in ED, inpatient units and compliance with transfer process through management of people, metrics and process improvement.

  • Activates the Hospital Surge/Capacity plan based on the guidelines established and works to initiate/activate/manage/close identified overflow patient care areas.

  • Monitors inpatient bed status and upgrades bed cleaning status via bed tracking system to expedite patient flow to a clean bed.

Requirements

  • A7:00am-7:30pm
  • Every other weekend.
  • ssociate's degree and graduate from accredited school of nursing. Four years experience in an acute care hospital setting to include minimum Telemetry experience. Critical Care experience preferred.

  • Need computer skills that include data input into multiple systems, working knowledge of windows based applications such as Microsoft Office skills, and ability to learn hospital based programs for access to lab results & bedtracking.

  • Valid NJ RN License.

  • Frequent physical demands include: Standing , Walking , Keyboard use/repetitive motion Occasional physical demands include: Sitting , Climbing (e.g., stairs or ladders) , Carry objects , Push/Pull , Twisting , Bending , Reaching forward , Reaching overhead , Squat/kneel/crawl , Wrist position deviation , Pinching/fine motor activities Continuous physical demands include: Talk or Hear Lifting Floor to Waist 15 lbs. Lifting Waist Level and Above 10 lbs.

Responsibilities * Performs safety and infection control techniques and functions in caring for the patient. * Uses correct techniques and follows standard precautions in applying patient restraints and in transferring patients. * Follows proper procedures and techniques to prevent falls and injuries. * Performs a variety of supportive and direct care functions (e.g. basic oxygen therapy, heat and cold applications, basic dressings, antiembolism and sequential pressure stockings). * Provides personal care (bathing, mouth care, AM/PM care, incontinence care) to patients. * Collects stool, urine and sputum specimens. * Provides for nutrition of patients (feeds, monitors tube feedings, calculates input and output, delivers nourishment, counts calories, weighs patients.) * Performs phlebotomy and stocks phlebotomy and IV trays for unit. * Performs EKG's and stocks and maintains supplies for the unit. Requirements * High school diploma or equivalency. * One year previous experience as a nurse aide in a health care setting OR less than one year experience and enrolled in an accredited school of nursing. * Certification as a Certified Nurse Assistant (CNA) preferred.
