The Patient Navigator coordinates services and guides patients across the continuum of care to help achieve the best possible quality, experience and affordability outcomes. The Patient Navigator works as part of an integrated care team that includes providers, clinic staff, social workers, financial counselors, community agencies and educators to effectively address the needs of patients in a collaborative, patient-centered environment.

Required Qualifications:
Preferred Qualifications:
Bachelor's Degree.
Education in a health-related field, e.g., public health, sociology, psychology or health education. Or equivalent educ/experience
5 years work experience as Medical Office SSC or Coordinator.
upon hire: Certification in an approved program of phlebotomy or medical assisting (for Postpartum Care Center, Mother Baby Clinic PPMC).
2 years Experience in the appropriate specialty setting.
Experience facilitating large group sessions and/or discussions.
